Relatie tussen de dosis, de plasmaconcentratie en de SERT-occupantie voor escitalopram: een PET studie met [11C]DASB in honden

Medicinale behandeling van gedragsgestoorde honden

Februari 2014. Een elfjarig meisje wordt aangevallen door een losgebroken bouvier. Deze aanval vond plaats zonder duidelijk oorzaak van een reactie van het meisje. De hond ging gewoon “door het lint” zoals men zegt. Blindelingse woede…

Gedragsstoornissen bij honden, iedereen heeft er wel al eens van gehoord in de media. Honden die mensen aanvallen en dan afgemaakt worden. Het probleem is dikwijls, net als bij mensen van psychische aard. In dit onderzoek gaan we grasduinen in de mogelijkheden om hier via geneesmiddelen een oplossing te kunnen bieden.

Selectieve serotonine reuptake inhibitoren worden algemeen aangewend in de behandeling van gedragsstoornissen bij honden. Serotonine is een stof die aangemaakt wordt in de hersenen in het presynaptisch neuron en vrijgegeven wordt in de synaptische spleet. Via een sleutel-slot mechanisme kan serotonine vervolgens binden op receptoren die een signaal uitzenden of op een transporter waardoor serotonine opnieuw opgenomen wordt in het presynaptisch neuron. Bij gedragsstoornissen is deze laatste werking verstoord. De aangewende geneesmiddelen zijn inhibitoren van deze transporter waardoor serotonine in de synaptische spleet blijft.

Om de serotonine transporter te visualiseren wordt gebruik gemaakt van een stof die aangemaakt wordt in het labo, een zogenaamde radiotracer. Deze wordt ingespoten in de honden waarna een PET-scan afgenomen wordt. Dit vindt zowel plaats vóór therapie met excitalopram, een selectieve serotonine reuptake inhibitor, als na therapie. Op die manier kan op de beelden een verschil in bindingssterkte waargenomen worden van de gebruikte radiotracer vóór therapie en na therapie met escitalopram. Alle honden krijgen hierbij een verschillende totale dagdosis toegediend.

Vervolgens worden de beelden verwerkt via speciale software waardoor de bezetting van de serotonine transporter bepaald kan worden. Uit een studie bij mensen zou een bezetting van 80% noodzakelijk zijn om een therapeutisch effect uit te lokken. De optimale dosis en het optimaal doseringsschema (hoeveel toedieningen per dag, hoeveel tijd tussen de verschillende toedieningen…) kunnen echter niet zomaar overgenomen worden uit studies bij mensen. Escitalopram verdwijnt namelijk veel sneller uit het lichaam van een hond waardoor meerdere toedieningen noodzakelijk zijn.

De optimale dagdosis wordt tenslotte bepaald met behulp van onderstaande grafiek. Daarop is te zien dat de optimale dagdosis bij 80% bezetting ongeveer 2 mg/kg bedraagt.

Bij de honden wordt eveneens een bloedstaal afgenomen op vijf verschillende momenten ter controle van de aanwezigheid van escitalopram (wanneer wordt de hoogste concentratie bereikt, wanneer de laagste concentratie…). Aan de hand hiervan kan een doseringsschema van 3 toedieningen per dag vooropgesteld worden waarbij escitalopram om de 6u toegediend wordt.

Laten we dan ook hopen dat dit onderzoek zal gevolgd worden en dat honden met gedragsstoornissen ook escitalopram zullen krijgen, zodat we op die manier kunnen voorkomen dat elfjarige meisjes zonder reden worden aangevallen en voor de rest van hun leven verminkt zijn. Want zoals  de bekende hondenfluisteraar Cesar Millan zegt: “Zorg dragen betekent niet dat je de hond toont wie de baas is, maar dat je verantwoordelijkheid neemt voor het levend wezen dat je in je leven hebt opgenomen.”
