Pijnlijke gevolgen: het eiwit waar men overkeek

Wereldwijd lijden ongeveer een miljard mensen aan chronische pijn. Deze last weegt zwaar door op zowel maatschappelijk als economisch vlak. Ondanks het grote beschikbare aanbod aan pijntherapieën kunnen ruim driehonderd miljoen mensen nog steeds niet geholpen worden. De nood is dus erg hoog aan de ontwikkeling van nieuwe pijnmedicatie. Recent groeide de interesse naar proNGF, een eiwit dat lang verwaarloosd werd in de context van pijn. Onderzoek wees uit dat dit eiwit misschien wel een nieuw aangrijpingspunt kan worden voor de behandeling van chronische pijn.

Chronische pijn

Wellicht kan niemand ontkennen dat pijn een van de meest onbehaaglijke sensaties vormt die ons lichaam kan ervaren. Onbehaaglijk maar wel noodzakelijk; het vermogen om pijn te voelen bevordert de overlevingskansen door ons op afstand te houden van schadelijke prikkels. In sommige gevallen kan pijn echter spontaan ontstaan in afwezigheid van een schadelijke stimulus. Wanneer dit gebeurt, dient pijn niet meer als een waarschuwingsmechanisme voor het lichaam, en gebruikt men de term ‘chronische pijn’.

Laten we ons even inbeelden dat we ononderbroken pijn ervaren die niet verholpen kan worden. Beeld je bijvoorbeeld in dat je een te diep afgescheurd, pietluttig velletje aan de rand van je vingernagel continu zou voelen. Dag in, dag uit. Je concentreren wordt lastiger, en elk leuk moment wordt verduisterd door het achterliggende pijngevoel. Een afgescheurd velletje is dan nog maar de meest minimale impact die we ons kunnen inbeelden. Het is dus niet verwonderlijk dat chronische pijnpatiënten die meestal continu veel hevigere pijn voelen, een daling van hun levenskwaliteit ondervinden en tevens een verhoogd risico op depressie hebben.

Buiten de last op persoonlijk vlak, is onbehandelbare chronische pijn ook een last voor de economie. De jaarlijkse kosten aan de gezondheidszorg en het verlies van werkproductiviteit lopen op tot 300 miljard euro in de Europese Unie alleen al. Aangezien oudere mensen vaker te kampen krijgen met chronische pijn, zullen door de vergrijzing deze kosten ongetwijfeld verder toenemen. De grote groep mensen die behoefte heeft aan verademing, samen met een poging de daarbij horende kosten te drukken, tonen aan dat de nood voor de ontwikkeling van nieuwe, doeltreffende medicatie zeer hoog is.

NGF, een tweesnijdend zwaard

Nerve Growth Factor (NGF) is een eiwit dat instaat voor de ontwikkeling, het onderhoud en het overleven van zenuwcellen. Naast deze belangrijke effecten heeft NGF ook nog een andere functie: het zorgt enerzijds voor het pijngevoel bij een kwetsuur en verlaagt anderzijds de pijndrempel. Op de plaats van een letsel brengt NGF veranderingen teweeg in de nabijgelegen zenuw waardoor deze gevoeliger wordt. Dit vernuftige mechanisme zorgt ervoor dat het verwonde lichaamsdeel minder gebruikt zal worden, zodat genezing bevorderd wordt. Omdat pijnzenuwen rechtstreeks verbonden zijn met het ruggenmerg, gebeuren op deze laatste locatie ook wijzigingen. Na herstel nemen deze veranderingen af, waardoor de verhoogde gevoeligheid weer verdwijnt. Nu komt echter de andere kant van de medaille. De impact van NGF is zo groot, dat in sommige gevallen de veranderingen in zenuwen en ruggenmerg kunnen aanhouden en uiteindelijk leiden tot het ontstaan van spontane chronische pijn.

Zoals veel eiwitten in het menselijk lichaam, wordt NGF als een groter ‘voorlopereiwit’ geproduceerd. Dit eiwit, ook wel proNGF genoemd, wordt ‘gesnoeid’, waardoor de kortere en definitieve vorm, NGF ontstaat. Aanvankelijk dacht men dat dit voorlopereiwit enkel de functie had te worden omgezet in NGF, maar niets blijkt minder waar te zijn. De verbazing was dan ook groot toen men ontdekte dat deze voorloper net als NGF ook kan inwerken op zenuwcellen, en er bovendien andere en vaak negatieve effecten op uitoefent. Wanneer bleek dat proNGF in hogere hoeveelheden aanwezig was in het centrale zenuwstelsel dan zijn volwassen vorm, NGF, ging er een belletje rinkelen.

Gezien de belangrijke rol van NGF in chronische pijn, begint onderzoek naar proNGF in pijn ook langzaam maar zeker op te komen. Het PAINCAGE project, gefinancierd door de Europese Commissie met als doel meer inzicht te vergaren in pijnmechanismen, heeft proNGF dan ook mee opgenomen als één van de onderzoeksonderwerpen met het oog op de ontwikkeling van nieuwe pijnmedicatie.

De invloed van proNGF

De eerste opmerkelijke bevindingen aangaande de rol van proNGF in pijn stromen ondertussen binnen. In een recente studie werden genetisch gemanipuleerde muizen, die meer proNGF bezitten dan in normale omstandigheden, onderzocht. Zo konden veranderingen gerelateerd aan pijn toegeschreven worden aan de invloed van een verhoging van proNGF.

Op genetisch vlak bleek dat proNGF genen kan aanschakelen die een rol spelen in het onderhouden van een pijngevoel. Verder bleek dat de cellen die instaan voor ontstekingsreacties en steun in het ruggenmerg van deze muizen, er helemaal anders uitzagen in vergelijking met normale muizen. Aangezien het ruggenmerg een erg belangrijke rol speelt in het onderhouden van pijn, zou het kunnen dat proNGF dit beïnvloedt via deze cellen.

Verschillen werden niet enkel waargenomen op moleculair niveau, maar ook op gedragsmatig vlak. Testen wezen uit dat muizen die te veel proNGF bezaten, gevoeliger werden aan pijnlijke stimuli bij veroudering, terwijl normale muizen hier minder gevoelig aan werden. Uit de combinatie van deze bevindingen werd besloten dat proNGF op eigen houtje wel degelijk een invloed kan uitoefenen op de gevoeligheid voor pijn.

De resultaten van deze studie zijn erg voorbarig, maar sporen bijkomend en diepgaand onderzoek aan. ProNGF, zo prominent aanwezig in ieder van ons, kwijnde jarenlang weg in vergetelheid, maar zou mogelijk een sleutel kunnen zijn in een nieuwe doorbraak bij de behandeling van chronische pijn. Het is een welkome en frisse wind die binnenwaait in de huidige, vastgeroeste en vaak ineffectieve pijntherapieën.

Investeren in verder onderzoek naar nieuwe aangrijpingspunten in chronische pijn is een must. De nood wordt bovendien versneld door de vergrijzing en de economische toenemende kosten. Misschien hoort u wel bij de zeven procent die ooit last zal krijgen van onbehandelbare chronische pijn. Op dat moment zal een behandeling die werkt, mogelijk ingrijpend op proNGF, even welkom zijn als een oase in een woestijn. Want het lijdt geen twijfel, niemand wil continu een gescheurd nagelvelletje voelen.
