U hebt het zeker al zelf ervaren. Wanneer er wordt gesproken over een zwangerschap, kent optimisme amper grenzen. Dit is zeker terecht, er groeit namelijk een nieuw leven in de vrouw. Een klein wondertje dat zich in negen maand ontspruit tot een levenslustige baby. Anderzijds heeft dit hele proces een invloed op zowel het psychische als fysische bestaan van de zwangere vrouw en niet steeds in de meest positieve zin. Naarmate de zwangerschap vordert en de baby dus groeit, is er sprake van gewichtstoename en verandert de lichaamsvorm van de vrouw. De heupen worden breder, striemen trekken lijnen over de buik en er kan acné optreden…

Het toilet is te ver

Dat het uiterlijk van de vrouw verandert tijdens de zwangerschap ziet iedereen. Hier stopt het echter niet. Het groeiende kind zal ook binnen in het lichaam zorgen voor verandering. In de studie ‘Pelvic floor dysfunction and its influence on body image and sexual function’ werden 146 zwangere, Vlaamse vrouwen bevraagd. In een van de topics werd gepeild naar de staat van incontinentie. Een persoon is incontinent wanneer hij of zij ongewild stoelgang of urine verliest en dit een sociaal of hygiënisch probleem vormt voor die persoon. Incontinentie kan optreden als gevolg van beschadiging van de bekkenbodemspieren. De bekkenbodem bestaat uit drie spierlagen die over elkaar liggen. Een van de functies van de bekkenbodem is het afsluiten van de endeldarm en de urinebuis. Bij onvoldoende afsluiting kan er dus ongewild urine of stoelgang vrijkomen. Bij de bevraagde vrouwen was 25% incontinent voor urine in het eerste trimester van de zwangerschap. Zwaarlijvige vrouwen hadden meer kans op incontinentie. Door het overgewicht wordt er namelijk meer druk uitgeoefend op de bekkenbodem waardoor deze kan falen in zijn sluitfunctie. Het percentage in de studie van incontinente vrouwen steeg tot een spectaculaire 65% in het derde en laatste trimester. Incontinentie bleek een groot deel van de bevraagde vrouwen te treffen. Uit vorige studies leek vaginale bevalling bij een vorige zwangerschap faciliterend te zijn voor incontinentie in een volgende. Dit werd in deze studie niet bevestigd. Uit de resultaten kwam wel naar voor dat vrouwen die in een vorige zwangerschap via een keizersnede zijn bevallen, gespaard bleven van incontinentie in het derde trimester. Dit komt doordat de bekkenbodemspieren niet werden belast door een vaginale bevalling. Laat dit nu echter geen reden zijn om met z’n allen massaal te kiezen voor een keizersnede. Hoewel de impact op de bekkenbodem minimaal is, brengt een keizersnede andere consequenties met zich mee.

De spiegel, niet mijn vriend

Een ander topic dat werd bevraagd was het zelfbeeld. Hoe ervaren zwangere vrouwen zelf hun lichaam… Uit vorige studies bleek dat het lichaamsbeeld niet sterk veranderde doorheen de zwangerschap. In de onderzochte Vlaamse populatie lag dat anders. Naargelang zwangerschap vorderde, scoorden de vrouwen hoger. Dit betekent dat de vrouwen het moeilijker kregen met het beeld dat ze van zichzelf vormden. Een gedachtegang die we kunnen volgen als we ons bedenken hoe sterk het lichaam verandert in de loop van de zwangerschap. Voor enkele vrouwen bleek de verstoring van hun lichaamsbeeld zo erg dat ze sociale aangelegenheden vermeden in het derde trimester. Zij gaven aan zelfs niet meer onder vrienden te willen komen, uit schaamte voor hun lichaam. Al gaat het hier om een extreme minderhheid, er zou meer aandacht moeten zijn voor het zelfbeeld tijdens de zwangerschap. Een negatief zelfbeeld kan immers leiden tot ongezonde gewoontes met als doel gewichtsverlies. Deze attitudes zijn niet enkel potentieel schadelijk voor de moeder maar kunnen ook de foetus in de buik schaden. Een verschil tussen het lichaamsbeeld van vrouwen met of zonder incontinentie werd niet gevonden. Om het negatief zelfbeeld volledig te wijten aan enkel de zwangerschap zou hier wel te kort door de bocht zijn.

Tussen de lakens

Een andere functie die de bekkenbodem heeft is een seksuele functie. Een goed getrainde bekkenbodem, kan bij bewuste aanspanning het seksueel genot van de vrouw verhogen. Bij incontinente vrouwen in het eerste trimester van de zwangerschap werd gevonden dat zij lager scoorden op de vragenlijst die het seksueel functioneren bevroeg. Een mooi voorbeeld van oorzaak en gevolg. Een functioneel minder goedwerkende bekkenbodem met als gevolg een minder goed seksueel functioneren. Hoewel incontinente vrouwen lager scoorden, bleken zij echter niet minder tevreden over hun seksueel leven dan de vrouwen die niet incontinent waren. In het algemeen werd geen enkele vrouw meer ontevreden over haar seksleven tijdens de zwangerschap, eindelijk weer wat goed nieuws! Anderzijds, meer dan de helft van de vrouwen die wel ontevreden waren, ondervonden relationele stress. Tot slot, naar het einde van de zwangerschap, had een op vijf geen enkele seksuele betrekking meer. Uit internationaal onderzoek blijkt de reden voornamelijk vermoeidheid te zijn.

Hoewel deze studie de keerzijde van de medaille laat zien, laat het u niet afschrikken. De zwangerschap is als een rollercoaster met veel happy hobbels door een roze wolk en crappy bobbels maar op het einde van de rit heeft u er wel een wondertje bij.
