“Het begon toen hij een jaar of zeventien was… hij gedroeg zich een beetje raar, anders dan anders. Misschien te wijten aan zijn puberteit? Langzaam aan verergerde dit en plots brak de hel los… zijn rare gedrag veranderde in roekeloos, gevaarlijk soms zelfs crimineel gedrag. Hij werd achterdochtig, vertrouwde zijn eigen ouders niet meer. Hele complottheorieën verzon hij om zijn gedrag te verantwoorden. Het hele gezin werd door hem geterroriseerd, het werd onmogelijk nog samen te leven… En toen viel de harde diagnose: een psychotische stoornis.”

 

En dan? Waar sta je als ouder wanneer je met deze diagnose geconfronteerd wordt? Je kind wordt opgenomen, gaat in behandeling. Als ouder krijg je dan gewoonlijk wel een vorm van psycho-educatie, een uitleg over de oorzaak en de gevolgen van de ziekte, de noodzaak van medicatie… Maar wat als je kind na de behandeling weer naar huis komt? Sinds een aantal decennia bestaat er immers zoiets als de ‘vermaatschappelijking van de zorg’ waarbij patiënten zoveel mogelijk in de maatschappij gelaten worden. Dit betekent dat de zorg voor de patiënt voor een groot deel op de schouders van familieleden terechtkomt.

 

Familieleden van een patiënt met een psychotische stoornis voelen zich vaak hulpeloos en de terugkeer naar huis van de patiënt kan een zware belasting vormen. Studies hier rond zijn vaak eenzijdig gebaseerd op ofwel de visie van de patiënt ofwel de visie van familieleden. Slechts enkele onderzoekers integreren beide visies. In de huidige studie werd er gesteld dat er een wisselwerking bestaat op het vlak van het gezin. Zowel de ouders als de patiënt oefenen een invloed uit op elkaar. Het onderzoek vond plaats bij gezinnen van patiënten met een psychose en controlegezinnen van niet-psychotische volwassenen. Er werd gesteld dat er vooral verschillen tussen beide groepen zouden zijn op niveau van het gezin en minder op niveau van het individu.

 

Drie begrippen, sociale steun, communicatie en hechting, werden nader onderzocht aan de hand van vier vragenlijsten.

Op basis van gegevens uit de literatuur leek het interessant na te gaan of  er een verschil is tussen klinische gezinnen en de controlegroep in de hoeveelheid sociale steun die ze ervaren. Hoe meer sociale steun een gezin ervaart, hoe minder zwaar de ziekte doorweegt. Bij gezinnen van psychotische patiënten zou dit een probleem zijn omdat ze vaak sociaal geïsoleerd raken. Ook zouden dergelijke gezinnen gekenmerkt worden door minder positieve communicatie en meer conflict. Minder adequate communicatie houdt bovendien in dat men vaak niet weet hoe de behoefte aan steun op een effectieve manier gecommuniceerd moet worden.

Wat het begrip hechting betreft gaan onderzoekers ervan uit dat een kind in relatie tot zijn ouders één van volgende drie hechtingsstijlen ontwikkelt: ‘veilige’, ‘vermijdende’ of ‘angstig-ambivalente’ hechting. Afhankelijk van de hechtingsstijl hanteert men een andere strategie van steun zoeken. Zo zal een ‘veilig’ gehecht individu meer sociale steun zoeken dan een ‘vermijdend’ of ‘angstig-ambivalent’ gehecht individu. In het onderzoek werd nagegaan of er een verschil in hechtingsstijlen kon gevonden worden tussen de gezinnen.

 

De resultaten toonden aan dat er vooral groepsverschillen gevonden werden op vlak van het gezin, en slechts in geringe mate op vlak van het individu. Er werd vastgesteld dat gezinnen van patiënten met een psychotische stoornis gekenmerkt werden door een hoger niveau van disfunctioneren in vergelijking met niet-klinische gezinnen. Dit uitte zich over verschillende domeinen gaande van meer conflict, minder adequaat probleemoplossend gedrag, gebrekkige communicatie tot afwezigheid van of gebrek aan sociale steun. Ook vertoonden klinische gezinnen de neiging meer vermijdend of angstig-ambivalent gehecht te zijn in tegenstelling tot controlegezinnen, die meer veilig gehecht waren.

 

De resultaten benadrukten eveneens dat het belangrijk is niet alleen te focussen op de problemen en conflicten binnen een gezin, maar vooral ook op het erkennen en versterken van de aanwezige positieve eigenschappen. Ook het belang van op maat gemaakte interventies voor het hele gezin kwam duidelijk naar voor. Een goede opvang, begeleiding en ondersteuning van de familie leidt tot een aanzienlijke verbetering van de vooruitzichten, niet alleen voor de patiënt maar voor het algemene klimaat binnen het gezin.

 

Bij een opname van een patiënt met een psychotische stoornis moeten we ervan uitgaan dat niet alleen een patiënt maar een heel systeem wordt opgenomen. Er bestaan reeds heel wat interventies gericht op de dichtste familie, toch worden ze nog te vaak niet aangeboden. Maar ook bij vrienden en kennissen is er een grote nood die niet altijd beantwoord wordt. Het is van belang ook aandacht te schenken aan het ruimere sociale netwerk, om zo het uiteenvallen van dit netwerk tegen te gaan. Hierbij moet in het achterhoofd gehouden worden dat een zekere teloorgang van het sociaal netwerk niet vermeden kan worden. Patiënten worden nagenoeg steeds teruggeworpen op de meest basale relaties in hun leven: familierelaties. En die moeten ondersteund worden!
