Opzet

In deze studie werd nagegaan of urinair continente en urinair incontinente vrouwen in staat zijn bewust hun bekkenbodemspieren te contraheren. Daarnaast werd ook nagegaan of de visuele observatie van een bekkenbodemspiercontractie na verbaal commando interbeoordelaars betrouwbaar is.

In de literatuur werden 3 studies (Bo et al., 1988; Bump et al., 1991 & Thompson et al, 2003) teruggevonden die beweerden dat slechts 50% van een groep incontinente vrouwen in staat zijn bewust de bekkenbodemspieren te contraheren.

  Materiaal en methode

De proefpersonen, gerekruteerd op de consultaties gynaecologie en urologie in het AZ VUB te Jette gedurende de periode van 1 juli 2004 tot 28 februari 2005, werden ingedeeld in een controle groep met continente vrouwen en een experimentele groep met incontinente vrouwen. Binnen deze groepen werden de proefpersonen via randomisatie aan een subgroep toegewezen.

De exclusiecriteria waren: (1) overactieve blaas, (2) mixed incontinentie, (3) <25jaar en >90jaar, (4) kinesitherapie <10jaar geleden, (5) urine incontinentie verhelpende operaties en (6) een ontsteking ter hoogte van de vagina.

Van alle proefpersonen werd een vragenlijst afgenomen, die de risicofactoren van urinaire incontinentie nagaat en werden er 3 onderzoeken, een verbaal commando met observatie, een verbaal commando met palpatie en de Q-tip test uitgevoerd.

Het verbaal commando werd, zowel bij observatie als bij palpatie, op 2 manieren gesteld: ‘Trek je spieren samen zoals je urine zou willen ophouden’ en ‘Trek je anus samen zoals je wind zou willen ophouden’. Bij de observatie van een bekkenbodemspiercontractie gold de minst zichtbare inwaartse beweging van het centrum tendineum als een contractie. Bij een duwbeweging naar buiten spreekt men van een valsalva manoeuvre. Via palpatie werd nagegaan of de bekkenbodemspiercontractie gepaard gaat met of zonder een lift van de vingers van de therapeut, geplaatst in de vagina.

Via de Q-tip test werd de proximale urethra en de blaashals mobiliteit nagegaan. Dit staafje wordt in het proximale deel van de urethra ingebracht en geeft bij ontspannen bekkenbodemspieren de positie van de urethra weer door een hoek te vormen met de lengteas van het lichaam (Schüssler et al., 1994). De Q-tip werd afgenomen in rust, waarbij de hoek 0° (neutraal), <30° (normaal) of >30° (hypermobiel) kan zijn. Bij intra-abdominale drukverhoging (vb. hoesten, niezen) kan het staafje onbeweeglijk blijven, een normale mobiliteit (<30°) of een hypermobiliteit (>30°) aangeven. Als laatste werd een bekkenbodemspiercontractie nagegaan (Karram et al., 1988).

 

Resultaten

Er werden 129 proefpersonen, met een gemiddelde leeftijd van 57 jaar (range 28-82), gewicht van 68 kg (range 47-103), lengte van 1m64 (range 1m50 -1m82) en BMI (Body Mass Index) van 25 (range 18-41), getest. Er zijn geen significante verschillen gevonden tussen de incontinente en de continente groep voor leeftijd, lichaamslengte, lichaamsgewicht en BMI.

Het onderzoek toont aan dat bij 101 van 129 proefpersonen een bekkenbodemspiercontractie wordt geobserveerd na verbaal commando. Na palpatie blijkt dat 105 van de geteste proefpersonen, waarvan 44 met lift van de vingers van de therapeut, hiertoe in staat zijn.

De studie laat verder zien dat 78% urinair continente en 84% urinair incontinente vrouwen bewust hun bekkenbodemspieren kunnen contraheren.

Tussen de beide groepen is geen significant verschil waarneembaar voor een bekkenbodemspiercontractie na verbaal commando. De resultaten geven wel een tendens weer waarbij de incontinente vrouwen vaker in staat zijn de bekkenbodemspieren samen te trekken dan de continente vrouwen. Wat betreft een bekkenbodemspiercontractie na palpatie werd eveneens geen significant verschil gevonden tussen de beide groepen.

Wanneer een bekkenbodemspiercontractie wordt geobserveerd, mag verwacht worden dat tijdens palpatie een contractie wordt gevoeld en omgekeerd. Wanneer een contractie wordt uitgevoerd na verbaal commando of bij palpatie kan verwacht worden dat de Q-tip test een contractie van de bekkenbodemspieren weergeeft en omgekeerd.

Om de mate van interbeoordelaars betrouwbaarheid van een observatie na verbaal commando na te gaan, onderzochten 2 onderzoekers, een gespecialiseerde kinesitherapeute en een studente kinesitherapie, 68 proefpersonen. De overeenkomst tussen de onderzoekers is bijna compleet en statistisch sterk significant (p=0.000). Dit indiceert dat observatie na verbaal commando zeer sterk inter-beoordelaars betrouwbaar is.

In het onderzoek werd een verband gevonden tussen hysterectomie en urinaire incontinentie en seksuele activiteit en urinaire incontinentie.

 

Discussie en conclusie

81% van de vrouwen zijn in staat de bekkenbodemspieren te contraheren. 85% van de incontinente vrouwen zijn hiertoe in staat en 78% van de continente vrouwen. In de studie is een tendens gevonden waarbij de incontinente vrouwen hun bekkenbodemspieren beter konden samentrekken dan de continente. Terwijl eigenlijk verwacht wordt dat continente vrouwen een stevigere bekkenbodem hebben omdat deze minder beschadigd zou zijn. Maar stel nu dat incontinente vrouwen zich meer bewust zijn van de bekkenbodem omdat ze, ten gevolge van het incontinent zijn, door allerlei ophoudmanoeuvres deze incontinentie trachten tegen te gaan. In de toekomst zouden nieuwe studies kunnen uitmaken of incontinente vrouwen inderdaad meer bewust zijn van de bekkenbodem dan de continente vrouwen.

Het onderzoek toont verder aan dat de observatie van een bekkenbodemspiercontractie na verbaal commando interbeoordelaars betrouwbaar is tussen 2 onderzoekers, wat deze eenvoudige techniek bruikbaar maakt voor iedereen.

Uit de studie blijkt dat vrouwen met een hysterectomie meer kans hebben op urinaire incontinentie, wat ook in de literatuur wordt teruggevonden (MØller et al. 2001, Milsom et al., 1993 & Melville et al. 2005). Hieruit kan dan ook besloten worden dat vrouwen met een hysterectomie eerder urinaire incontinentie zullen ontwikkelen dan vrouwen zonder hysterectomie en omgekeerd.

De resultaten van ons onderzoek geven een verband weer tussen de mate van seksuele activiteit en urinaire incontinentie. Vrouwen die urinaire incontinent zijn hebben minder vaak seksuele betrekkingen en mogelijks omgekeerd. Het is nog niet geweten of urinaire incontinentie aanleiding geeft tot verminderde seksuele betrekking of verminderde seksuele betrekking aanleiding geeft tot urinaire incontinentie. In de literatuur werden geen studies teruggevonden die deze hypothese konden bevestigen of weerleggen.
