Executieve functies in het dagelijks leven bij kinderen met dyslexie en dyscalculie: Inhibitie bij kinderen tussen 8 en 12 jaar met een comorbide leerstoornis

 

"Ik kon er niets aan doen!" (Over)leven in een wereld vol verwachtingen en prikkels.
Kinderen leven vandaag in een wereld vol verwachtingen en prikkels. Verwachtingen dat ze opletten in de les en ze zich zullen gedragen tijdens een restaurantbezoek. Verwachtingen dat ze spontaan emoties kunnen uiten en hun woede kunnen beheersen. Kortom, verwachtingen dat ze zich nu en op latere leeftijd sociaal aanvaardbaar zullen gedragen. Verschillende prikkels kunnen er echter voor zorgen dat kinderen niet aan die verwachtingen voldoen, maar dat ze impulsief en ongeremd reageren. Slechte opvoeding, of is er iets meer aan de hand?
 
We beschikken allemaal over een aantal cognitieve vaardigheden die ons helpen om ons aan nieuwe of veranderende situaties aan te passen. Deze vaardigheden worden aangeduid als executieve functies en stellen ons in staat om flexibel te reageren zonder slaaf te worden van onze omgeving. Via onze zintuigen ervaren we een prikkel die door ons executief vermogen wordt verwerkt en omgezet in een motorisch antwoord hierop. Inhibitieproblemen onderbreken deze cyclus. Ze kunnen er voor zorgen dat kinderen niet kunnen opletten in de les en ze zich impulsief gedragen in een restaurant. Er is dus iets meer aan de hand.
De werking van ons inhibitiemechanisme is grotendeels te herleiden tot 3 processen. Allereerst blokt het irrelevante informatie waardoor we ons beter kunnen focussen. Ten tweede onderdrukt het dominante of relatief automatische reacties waardoor impulsief gedrag wordt beperkt. Ten derde zorgt het er voor dat een individu bij het uitvoeren van een taak niet afgeleid wordt door andere impulsen of gebeurtenissen.
Inhibitieproblemen verstoren deze functie en worden door factoren zoals dyslexie en dyscalculie nog versterkt. Met dyslexie bedoelen we een hardnekkig probleem met het aanleren enerzijds en het accuraat en/of vlot toepassen van het lezen en/of spellen op woordniveau, anderzijds. Onderzoek wijst uit dat taken die inhibitie stimuleren, er voor zorgen dat de leessnelheid en de accuraatheid bij dyslectische kinderen stijgt. Kinderen met dyscalculie hebben dan weer problemen om zich de wiskundige vaardigheden eigen te maken. Opnieuw speelt het inhibitiesysteem zijn rol. Rekentaken vereisen immers het tegelijk vasthouden van relevante informatie enerzijds, en het verwijderen van irrelevante anderzijds. Zo zal het bij het oplossen van een vraagstuk helpen om onnodige gegevens te blokkeren waardoor de juiste oplossing kan gevonden worden. Wetende dat dyslexie en dyscalculie elk afzonderlijk sterk gecorreleerd zijn met het inhibitiemechanisme, kan men zich de vraag stellen wat de invloed zal zijn bij kinderen met een comorbide leerstoornis. Met comorbiditeit wordt het gelijktijdig of opeenvolgend voorkomen van twee of meer aandoeningen bij een individu bedoeld. Onderzoek wees aan dat dyslexie en dyscalculie vaak samen voorkomen. In mijn thesis heb ik mij gefocust hoe de comorbiditeit van deze twee leerstoornissen een invloed heeft op het inhibitievermogen van 77 kinderen tussen 8 en 12 jaar, waarvan 32 een comorbide leerstoornis hebben en 25 kinderen een geïsoleerde. De controlegroep bestond uit 20 kinderen zonder leerstoornissen.
 
Volgens de literatuur zijn abstracte tests belangrijk om een wetenschappelijke basis aan de conclusies te geven. Om aan deze eis te voldoen, werden inhibitieproblemen onderzocht in een experimentele setting. In mijn onderzoek deed ik daarom een beroep op de Stroop taken zoals de Stroop Kleur. In die laatste test dient een proefpersoon eerst de kleur van een rechthoek te benoemen (groen, rood of blauw). Nadien dient hij de kleur te benoemen waarin een bepaald woord geschreven staat. Het woord dat hij dient te lezen komt in deze conditie overeen met de kleur zelf (vb. ‘groen’ geschreven in het groen). Tenslotte krijgt de proefpersoon een groengekleurd woord ‘rood’ te zien, en wordt opnieuw gevraagd de kleur van het woord aan te geven. Een goed werkend inhibitiesysteem zal er voor zorgen dat een respondent ‘groen’ zegt en niet impulsief ‘rood’ aangeeft. Uit mijn onderzoek blijkt dat kinderen uit de comorbide groep op de Stroop taken beduidend meer fouten maakt dan de controle groep, maar niet dan de geïsoleerde groepen. Omdat de comorbide leerstoornis de additieve combinatie is van dyslexie en dyscalculie, is het inhibitieprobleem prominenter aanwezig. Dat blijkt ook wanneer we de tijd die nodig was om de Stroop taken uit te voeren, in rekening nemen. Voornamelijk met de Stroop Kleur-Woord, die eerder werd geïllustreerd, bleek de comorbide groep problemen te hebben. Deze tragere reactietijd bij de comorbide groep is te wijten aan problemen met inhibitie.
Aangezien het functioneren in het dagelijks leven minstens even belangrijk is, mag men zich niet enkel op deze tests baseren om inhibitie in kaart te brengen. Leerkrachten kunnen inschatten of een kind zich kan concentreren in de les en ouders weten of hun jongste zich gedraagt in het restaurant. Om die meningen te vatten, maakte ik gebruik van de BRIEF-vragenlijst (Behavior Rating Inventory of Executive Function). Hieruit blijkt dat ouders van kinderen met een comorbide stoornis niet vinden dat zij meer inhibitieproblemen hebben dan kinderen met een geïsoleerde stoornis of kinderen zonder leerstoornis. Volgens de leerkrachten is dat verschil er wel. Dit is niet geheel onverwacht daar kinderen op school veel meer beroep moeten doen op inhibitie dan thuis het geval is, en problemen dus sneller zullen opvallen in een schoolse context.
 
De inhibitieproblemen komen dus zowel naar voor in de experimentele setting als in het dagelijkse leven. Wel is het zo dat beide benaderingen verschillende manifestaties van het probleem vaststellen. Er is immers geen relatie tussen de scores op de BRIEF-vragenlijsten en de gestandaardiseerde Stroop taken. Beide instrumenten beslaan dus andere aspecten van executief functioneren. Wil men een juiste diagnose stellen, dan zal er een veelzijdige benadering nodig zijn.
Het therapeutisch handelen zal hier eveneens aandacht moeten aan besteden. Bij het behandelen van kinderen met comorbide dyslexie en dyscalculie dienen bovendien zowel het lezen als het rekenen aangepakt te worden. Om de inhibitieproblemen tijdens de behandeling te gaan trainen, kan de moeilijkheidsgraad van de oefeningen gradueel worden opgebouwd. Bij het oplossen van vraagstukken kan er in een eerste fase voor gekozen worden om geen overbodige informatie aan te bieden, zodat deze dan ook niet onderdrukt dient te worden. Nadien kan de moeilijkheidsgraad worden opgebouwd. Bij het lezen kan een gelijkaardige denkpiste gevolgd worden. bij de behandeling van de leerstoornissen meteen oefeningen worden gebruikt die veel beroep doen op het inhibitiemechanisme, kan dit nefast zijn voor de motivatie van het kind. Het is duidelijk dat de inhibitieproblemen bij kinderen meerdere oorzaken, maar ook meerdere oplossingen kent. Afgeleid zijn in de les of ongepast reageren in een restaurant is niet alleen een kwestie van opvoeding. Er is duidelijk meer aan de hand.
