Prognostic and pathobiologic relevance of morphological and immunohistochemical markers in ductal carcinoma in situ of the breast

Volgens de recentste cijfers en statistieken van de Stichting Kankerregister werd in 2008 borstkanker vastgesteld bij 5.528 Vlaamse vrouwen. Ongeveer een kwart van hen zal sterven aan deze ziekte. Dit maakt borstkanker tot de meest voorkomende vorm van kanker, en tevens tot de belangrijkste oorzaak van kankersterfte bij vrouwen in Vlaanderen.

Wereldwijd wordt onderzoek gevoerd naar borstkanker, met als doel het ontstaan van deze kanker beter te begrijpen en nieuwe behandelingen te ontwikkelen. Aanvankelijk focusten onderzoekers op de kankercellen zelf. Echter, de afgelopen jaren werd duidelijk dat ook het omringende gezonde weefsel rond de kankercellen een belangrijke rol speelt in de evolutie van borstkanker. Dit ogenschijnlijk normale weefsel rondom de borstkanker noemt men stroma. Verschillende interacties tussen de kankercellen en het stroma zouden bepalen of een lokale borstkanker zich al dan niet verder door het lichaam verspreidt.

In dit onderzoek werd het stroma bestudeerd bij 61 patiënten met borstkanker in een vroeg stadium. Het doel van deze analyse bestond erin een bepaalde eigenschap in het stroma te identificeren, die zou toelaten beter te voorspellen welke patiënten zullen hervallen na behandeling. Het resultaat lijkt bijzonder veelbelovend: één bepaald eiwit, decorine, komt duidelijk minder voor in het stroma van borstkankers die er zeer kwaadaardig uitzien.

De meeste borstkankers ontstaan uit de cellen die de afvoergangen van de melkklier bekleden. Eén van deze cellen wordt kwaadaardig en begint nieuwe cellen af te splitsen, die ook kwaadaardig zijn. Zo ontstaat een kankergezwel. Ongeveer één op vijf borstkankers bevindt zich op het moment van de diagnose in een vroeg stadium. In dit vroege stadium bevindt het kwaadaardig gezwel zich nog ter plaatse in de afvoergang van de melkklier. Het gaat dan om een lokale borstkanker. Het gevaar bestaat echter dat de kankercellen niet ter plaatse blijven, maar buiten de melkkliergang groeien, in het gezonde stroma. Dit proces, waarbij kankercellen in het stroma migreren, noemt men invasie. De borstkanker is dan niet meer lokaal, maar invasief. Wanneer deze kankercellen de bloed- of lymfevaten bereiken, kunnen ze via het bloed of lymfevocht over het hele lichaam verspreiden en uitzaaiingen vormen, bijvoorbeeld in de lever, de longen of de hersenen. Deze uitzaaiingen zijn levensbedreigend.

Omwille van het risico op het ontwikkelen van een invasieve kanker, bestaat de behandeling van een lokale borstkanker uit het wegsnijden van het hele gezwel. Bij een lokale borstkanker van beperkte grootte wordt doorgaans voor een borstsparende operatie gekozen, al dan niet aangevuld met bestraling. Toch zal één op vijf vrouwen hervallen na borstsparende chirurgie. Bij herval kan het opnieuw om een lokale borstkanker gaan, maar in de helft van de gevallen gaat het om een invasieve borstkanker, met risico op levensbedreigende uitzaaiingen.

Momenteel is het nog steeds niet evident te voorspellen welke vrouw zal hervallen na een borstsparende operatie. In dit onderzoek werd gefocust op het stroma rond borstkanker in een vroeg stadium, om een voorspellende merker te identificeren.

In een eerste fase van het onderzoek werd het weefsel van 61 vrouwen met lokale borstkanker bestudeerd met behulp van een microscoop. Alle kankers werden op basis van de pathologische indeling verdeeld in twee groepen. Deze indeling wordt bepaald aan de hand van de hoeveelheid afwijkingen in de kwaadaardige cellen. Zo ontstond een groep met hooggradige lokale borstkankers (veel abnormaliteiten in de kankercellen) en een groep met laaggradige lokale borstkankers (minder uitgesproken afwijkingen in de cellen). Deze pathologische indeling weerspiegelt de kwaadaardigheid van de kanker. Aangezien geen gegevens beschikbaar waren over eventuele herval, werd deze pathologische indeling gebruikt als een vervangende merker voor de prognose van de 61 patiënten. Uit eerdere studies is immers reeds afgeleid dat vrouwen met hooggradige lokale borstkanker een groter risico lopen om te hervallen na een borstsparende operatie.

In een tweede fase van het microscopisch onderzoek werd de vorm van het stroma rond de lokale borstkanker nagegaan. Het stroma is doorgaans opgebouwd uit dicht op elkaar liggende bindweefselvezels. Echter, bij sommige patiënten waren rondom de kanker minder bindweefselvezels aanwezig en was de samenstelling van het stroma veranderd, wat in vaktermen 'myxoid stroma' wordt genoemd. Ook de hoeveelheid ontstekingscellen in het stroma werd onderzocht. Uit deze evaluatie bleek dat hooggradige lokale borstkankers opvallend vaker omringd werden door een myxoid stroma, waarin ook nog eens veel meer ontstekingscellen voorkwamen. Deze twee eigenschappen zouden gebruikt kunnen worden om beter in te schatten wat de kans is op herval na een borstsparende operatie.

Vervolgens werd onderzocht of er een verband bestond tussen de aanwezigheid van negen verschillende eiwitten enerzijds, en de pathologische indeling anderzijds. Dit verband bleek zeer sterk aanwezig voor één van de negen onderzochte eiwitten, namelijk decorine. Dit decorine is een relatief klein eiwit dat vastzit op de collageenvezels in het bindweefsel. Decorine is meer dan louter 'decoratie' van het bindweefsel. Het zorgt ervoor, samen met een heleboel andere eiwitten, dat deze collageenvezels een hecht netwerk vormen, zodat het bindweefsel steun geeft aan de verschillende organen in ons lichaam, waaronder de borst. Uit dit onderzoek kwam zeer duidelijk naar voor dat de hoeveelheid decorine in het stroma sterk verminderd is bij zestig procent van de hooggradige lokale borstkankers, terwijl dit bij laaggradige lokale borstkankers slechts acht procent bedraagt.

Momenteel kan dit verband nog niet met zekerheid verklaard worden. Een mogelijke hypothese is dat hooggradige lokale borstkankers een stof produceren die decorine in het omringende stroma afbreekt. Daardoor wordt het netwerk van collageenvezels in het stroma minder hecht. De kwaadaardige kankercellen hebben het dan makkelijker om uit de afvoergang van de melkklier te treden en doorheen het stroma te migreren. Een vermindering van decorine in het stroma bevordert dus het proces van invasie!

Dit onderzoek bij 61 vrouwen met lokale borstkanker wees uit dat de vorm van het stroma, de omvang van ontsteking en de hoeveelheid decorine in het stroma gerelateerd zijn aan de mate van kwaadaardigheid van de kanker. Decorine in het bijzonder komt hier naar voor als een veelbelovende krachtige voorspellende merker bij het inschatten van de prognose van borstkanker in een vroeg stadium. In de toekomst zal decorine onder de loep worden genomen in een grotere groep patiëntes met lokale borstkanker, met en zonder herval na behandeling. Dit moet op termijn leiden tot een voorspellende merker die door de behandelende arts kan worden toegepast in de dagelijkse praktijk.
