Stigmatisering van Psychiatrische Patiënten door Huisartsen en Studenten Geneeskunde

Stigma misleidt de huisarts bij de zorg voor psychiatrische patiënten.

 Psychiatrische patiënten werden lang bestempeld als demonen, zotten, krank- of zwakzinnigen. Men ving hen op in psychiatrische ‘gevangenissen’ ver weg van de ‘normale’ wereld, waar ze experimentele behandelingen moesten ondergaan. Zo namen artsen bijvoorbeeld een stuk van de hersenen weg. Gelukkig is die tijd voorbij en worden patiënten met psychische problemen vandaag verzorgd op psychiatrische afdelingen, in hetzelfde ziekenhuis als ‘normale’ patiënten.Toch wil dit niet zeggen dat Jan Modaal zijn psychische gezondheidsproblemen met anderen deelt. Daarom bestaan er in Vlaanderen organisaties, zoals ‘Te Gek’, die grote sensibiliseringscampagnes op touw zetten om het taboe rond psychiatrische aandoeningen en geestelijke gezondheidszorg te doorbreken. En dat dit vandaag nog steeds zeer actueel en nodig is, bewijst ook de zesdelige documentairereeks van Annick Ruyts op Canvas.Ook in de gezondheidssector speelt dit taboe een rol. Bijna dagelijks zeggen patiënten ‘Dokter, ik vind het zo zwak dat ik die antidepressiva moet nemen’ of ‘Niemand in mijn familie weet het’. Zou het kunnen dat deze negatieve houding tot een slechtere gezondheidszorg voor psychiatrische patiënten leidt? En is dit dan stigma?Tot begin 2010 bestond over stigmatisering van psychiatrische patiënten door huisartsen slechts zeer beperkt Vlaams onderzoek. Het leek dan ook een ideaal thesisonderwerp voor Katrien Adriaensen, huisarts in opleiding. Maar wat is stigma juist? In de literatuur wordt stigma omschreven als: ‘een proces van diskwalificatie waarbij een normaal persoon gereduceerd wordt tot iemand waarmee iets mis is’.De internationale literatuur staat bol van frappante vaststellingen over de stigmatisering van psychiatrische patiënten door huisartsen. Zo stigmatiseren huisartsen psychiatrische patiënten net zoals de algemene bevolking, en laten ze zich door dit stigma misleiden bij de zorg voor deze patiënten. Hoewel psychiatrische patiënten vaker op consultatie komen bij de huisarts, is hun fysieke gezondheid minder goed dan die van andere patiënten. Ze roken frequenter en zijn vaker te zwaar, waardoor ze meer long- en hartproblemen ontwikkelen. Ze hebben niet zelden een hoge bloeddruk of suikerziekte. Hoe komt het dat huisartsen dit niet zien of aanpakken?De literatuur geeft hiervoor twee belangrijke verklaringen. Enerzijds blijkt dat huisartsen bepaalde klachten van psychiatrische patiënten, bijvoorbeeld hoofdpijn, makkelijk toeschrijven aan hun psychiatrische aandoening. Deze klacht onderzoeken ze dan minder uitgebreid dan bij andere patiënten. Ze vragen minder technische onderzoeken aan, zoals een bloedname of een foto van de longen. Dit heet ‘diagnostic overshadowing’. Anderzijds blijkt dat psychiatrische patiënten hun fysieke klachten niet snel spontaan vermelden, terwijl ze daar tijdens de regelmatige consultaties om herhaalvoorschriften voor antidepressiva of antipsychotica de kans toe hebben.Daarnaast spelen verschillende praktijkorganisatorische elementen een rol in de moeilijk zorg voor psychiatrische patiënten. Zo vindt vijfenzestig procent van de huisartsen dat er tijdens de opleiding te weinig tijd wordt besteed aan psychiatrie. Voorts vinden ze de vaak langdurige consultaties moeilijk inplanbaar in de dagelijkse praktijk. Ten slotte verloopt de samenwerking met de gespecialiseerde gezondheidszorg stroef.Ook de studenten geneeskunde hebben, als dokters van morgen, een negatieve houding ten aanzien van psychiatrische patiënten en ‘de psychiatrie’ in het algemeen. Ze zien medestudenten, die een bijzondere interesse koesteren voor psychiatrie, als vreemd, excentriek, neurotisch, wetenschappelijk zwak en niet helder of logisch denkend. Omdat er in België nog geen onderzoek gebeurde naar ‘stigmatisering van psychiatrische patiënten door huisartsen’ werd een focusgroeponderzoek verricht in de huisartsenregio Mortsel. Bij die onderzoeksmethode discussiëren groepjes van 10 tot 15 huisartsen over een onderwerp, waarbij onderzoekers nadien de gedane uitspraken in thema’s verdelen. Het doel van dit onderzoek is de houding van de Vlaamse huisarts in de opvang van psychiatrische patiënten tijdens de dagelijkse praktijk na te gaan.De Vlaamse huisarts ervaart inderdaad stigma tijdens de dagelijkse praktijk, en definieert het als: vooroordelen, stempel, een negatief label of een extra kwetsbaarheid. Ze ondervinden allerlei problemen in de zorg voor psychiatrische patiënten, zoals: een gebrek aan kennis en ervaring, eigen emoties bijvoorbeeld een gebrek aan interesse of frustratie en het feit dat psychiatrische patiënten vaak veel tijd vragen. Daarnaast vinden huisartsen het moeilijker communiceren met psychiatrische patiënten en verloopt de opbouw van een vertrouwensrelatie anders. Tot slot blijkt het moeilijk samenwerken met psychiaters. Omdat huisartsen problemen bij de zorg voor psychiatrische patiënten ondervinden, maken ze een onderscheid tussen ‘de psychiatrische patiënt’ en ‘de niet-psychiatrische patiënt’ en passen hun consultatievoering hieraan aan. De huisarts probeert duidelijke afspraken te maken met ‘de psychiatrische patiënt’ en verwijst hen sneller door naar een specialist (psychiater of psycholoog). Een gevolg van stigma blijkt het anders nadenken over - en aanvragen van - technische onderzoeken. Als mogelijke oplossingen voor het probleem van stigmatisering suggereren huisartsen extra opleiding en een vergoeding voor langdurige gesprekken. Uit bovenstaande blijkt dat stigma de huisarts inderdaad misleidt tijdens de dagelijkse praktijk. Aanpak van dit stigma kan dus leiden tot een betere kwaliteit van de gezondheidszorg voor psychiatrische patiënten. Toch is er onvoldoende geweten over hoe we dit stigma het best aanpakken, en is in eerste instantie verder onderzoek nodig. Tijdens het focusgroeponderzoek deden de Vlaamse huisartsen enkele suggesties om zich meer ondersteund te voelen bij de zorg voor psychiatrische patiënten. Zo zou er een betere samenwerking moeten komen tussen huisartsen en psychiaters. Ook extra opleiding is nodig, iets waarmee men het best al kan beginnen bij de dokters van morgen, zijnde de studenten geneeskunde. Daarnaast vindt men langdurige consultaties moeilijk inplanbaar in de dagelijkse praktijk en zou ook hier ondersteuning, bijvoorbeeld van een psychiatrisch verpleegkundige, kunnen helpen. Ten slotte zou een algemene aanpak van de stigmatisering van psychiatrische patiënten in de maatschappij kunnen leiden tot een brede acceptatie, en op die manier ook tot een kwaliteitsverbetering binnen de gezondheidszorg. Er blijft dus nog veel werk aan de winkel.
