ADHD en langdurig medicatiegebruik. Belevingsonderzoek bij ouders van kinderen met ADHD.

 Langdurig schoolmedicatie voor ADHD. De oplossing?

Hedendaags volgen veel kinderen met ADHD al meerdere jaren een behandeling met medicatie. Hun ouders nemen de beslissingen over het opstarten en uitvoeren van deze behandeling. Onderzoek wijst uit dat ouders na vele jaren bezorgdheden en twijfels blijven kennen over het medicatiegebruik. De hoofreden waarom de behandeling wordt opgestart en verdergezet, is de school. De psychiater is hierbij de persoon die de medicatie voorschrijft en opvolgt. Overleg tussen de psychiater en de school vindt echter amper plaats. In de toekomst hopen ouders op een nieuwe behandeling voor ADHD. Ze vinden het belangrijk dat deze behandeling geen nevenwerkingen heeft en bovendien op langere termijn geen invloed vertoont op de gezondheid van het kind.

Het onderzoek werd uitgevoerd bij zeventien ouders van kinderen met ADHD. Alle kinderen volg(d)en langer dan twee jaar een behandeling met medicatie. De kinderen waren tussen de zes en achttien jaar oud. Uit het onderzoek blijkt dat de medicatie voordelen biedt, zoals een afname van de ADHD-symptomen en het verbeteren van het emotioneel welbevinden. Opvallend hierbij is dat de voordelen voornamelijk geplaatst worden in de schoolcontext. De medicatie zorgt ervoor dat het kind beter functioneert en presteert op school. De negatieve ervaringen ten gevolge van opmerkingen van de leerkrachten verminderen. Dit heeft een positief effect op het welzijn van het kind.

Mama Ibrahim: “Vooral zijn concentratie op school. Dat heeft de grote doorslag gegeven... Hij kreeg continu onder zijn voeten... Enkel eigenlijk het schoolse, dat lukt veel beter.”

De school is een instelling die de ouders voornamelijk wijst op het probleemgedrag van het kind en verwacht dat de ouders dit gedrag aanpakken. Ouders geven de school aan als de hoofdreden om de behandeling met psychofarmaca te starten en gedurende meerdere jaren verder te zetten. Toch verkiezen ouders om het kind geen medicatie te geven. Zij beschouwen deze niet als de geschikte oplossing in de aanpak van ADHD en wensen dat het kind er snel met zou kunnen stoppen. In de toekomst hopen zij op de komst van een nieuwe behandeling die de voordelen van de medicatie biedt, maar niet de nadelen. Dit omwille van de bezorgdheid over de negatieve effecten van het medicatiegebruik. De bezorgdheden over de huidige medicatie gaan eerst en vooral over de directe bijwerkingen, zoals slaapstoornissen en het heviger optreden van de ADHD-symptomen wanneer de medicatie uitwerkt (reboundeffect). Daarnaast zijn ouders ongerust over de gevolgen op volwassen leeftijd. Op de vraag wat deze gevolgen op latere leeftijd zijn, heeft de wetenschap immers nog geen antwoord. De derde bekommernis is dat de medicatie als drugs beschreven worden. Ze hebben schrik dat hun kind later zal overschakelen op een andere drug ter vervanging van de medicatie. Het niet overtuigd zijn van de huidige behandeling met medicatie voor ADHD verklaart waarom ouders blijven zoeken naar alternatieve behandelingen en deze uittesten. In de praktijk wordt de medicatie dan ook vaak gecombineerd met andere behandelingen.

Ouders nemen bewust het besluit om met de behandeling met medicatie te starten en deze verder te zetten. Zij zoeken betrouwbare informatie hierover. Deze vinden zij vooral bij de psychiater en bij personen die een expertise hebben over ADHD. De psychiater start de medicatie op en volgt het verloop op. Ouders overleggen met de psychiater over de behandeling met medicatie en krijgen hierbij inspraak. Zij zijn zich ervan bewust het laatste woord te hebben. De beslissingen die worden genomen in overleg met de psychiater voeren ouders niet steeds correct uit, wat ertoe leidt dat de therapietrouw in de praktijk vaak erg laag is. De argumenten die ouders hiervoor aanhalen, komen overeen met hun visie op het medicatiegebruik. Het overleg tussen de ouders en de school over het medicatiegebruik van het kind vindt meestal enkel plaats tijdens de opstartfase. Na verloop van tijd blijkt de medicatie nog maar een weinig besproken onderwerp met de school. Enkel wanneer het kind opnieuw gedragsproblemen stelt in de school, wordt dit opnieuw besproken. De school verwacht dat de ouders de gedragsproblemen van het kind aanpakken via de medicatie. Overleg over andere oplossingen of aanpakken voor de gedragsproblemen vindt weinig plaats. Contact tussen de psychiater en de school is zo goed als afwezig, en dit zowel bij het opstarten als in het verdere verloop van de behandeling. Dit is een opmerkelijk gegeven aangezien het kind de medicatie hoofdzakelijk neemt om te kunnen functioneren in de school en de psychiater verantwoordelijk is voor een goede afstemming van de medicatie.

Hoe jonger de leeftijd van het kind, hoe minder inspraak hij/zij heeft in het medicatiegebruik. Op deze leeftijd observeren ouders wel het gedrag van hun kind en vragen hoe hij/zij zich voelt bij de medicatie. Hiermee houdt de ouder rekening in het overleg met de psychiater en bij het nemen van beslissingen. Hoe ouder het kind, hoe meer bekwaam hij/zij wordt geacht en hoe meer inspraak hij/zij krijgt in de besluitvorming. Ook verwachten ouders dat het kind meer verantwoordelijkheid opneemt. De beslissingen waarbij het oudere kind betrokken wordt, gaan over het al dan niet volgen van een behandeling met medicatie, de medicatiedosis, etc. Op deze leeftijd neemt hij/zij meestal zelf de medicatie in en dient hiervoor geen toestemming aan een volwassene te vragen. De mate van overleg over de medicatie is afhankelijk van het belang dat de ouders hieraan hechten en de houding van het kind. Sommige kinderen stoppen de behandeling met medicatie op eigen houtje. Ze bespreken dit nauwelijks met hun ouders en overleggen dit niet met de psychiater.

Ouders waarvan het kind met ADHD al meerdere jaren medicatie neemt, hopen dus in de toekomst op een nieuwe behandeling die, net zoals de medicatie, de gedragsproblemen op de school vermindert. Bij de huidige toegepaste behandeling met medicatie bij het kind met ADHD is een betere opvolging aangewezen. Hierbij is overleg tussen de psychiater en de school essentieel. De hoofdreden waarom ouders de behandeling met medicatie bij hun kind opstarten en verderzetten, betreft immers de school.
