Psychodiagnostic assessment of internationally adopted children referred to the UZ Brussel CAMHS
Nood aan opvolging van adoptiegezinnen na adoptie
BRUSSEL – Op de Psychiatrische Afdeling voor infants, kinderen en adolescenten (PAika) van het UZ Brussel werd recent een onderzoek over adoptiekinderen afgerond. Uit de studie blijkt dat deze kinderen (en hun adoptiegezin) dikwijls met complexe problemen te maken krijgen, waaronder hechtingsproblemen. Ondanks uitgebreide voorbereiding van de adoptieouders, blijven veel gezinnen op hun honger zitten wat betreft de opvolging na adoptie.
Bij de studie werden alle kinderen betrokken die vanuit het buitenland werden geadopteerd en op de dienst kinderpsychiatrie van het UZ Brussel werden geëvalueerd (samen met hun adoptiegezin) tussen 2005 en 2012.
Volgens dr. Sam Bonduelle, kinderpsychiater in opleiding aan de Vrije Universiteit Brussel (VUB) die meewerkte aan de studie, gaat het om een heel diverse groep kinderen: “We zagen zowel jongens als meisjes, tussen 3 en 17 jaar oud, afkomstig uit 12 verschillende landen. De kinderen en jongeren kwamen om allerlei redenen naar PAika, sommigen eerder uit voorzorg, anderen met een dringende, gegronde hulpvraag.”
Vooral de adoptieouders zochten hulp, maar ook andere hulpverleners (huisarts, kinderarts, psycholoog), leerkrachten en CLB’s stuurden de kinderen regelmatig door. De kinderen werden het vaakst aangemeld omwille van gedragsmoeilijkheden, maar ook emotionele en sociale problemen, moeilijkheden op school, gebrek aan concentratie, worstelen met identiteit, opvoedingsproblemen, angststoornissen, enz. waren veel voorkomende aanmeldingsredenen.
Het is al langer geweten dat adoptiekinderen relatief vaak verwezen worden naar kinderpsychologen en –psychiaters. Nochtans hebben recente grootschalige studies aangetoond dat de meeste adoptiekinderen een belangrijke inhaalbeweging doormaken na hun adoptie ten opzichte van niet-geadopteerde kinderen. “Inderdaad, bij de meerderheid van de adoptiekinderen stellen er zich in feite geen grote problemen. Toch blijven ze als groep gemiddeld een beetje achterop hinken in een aantal domeinen. Zo doen ze het wat minder goed op school en hebben ze vaker hechtingsproblemen. Het zijn natuurlijk die kinderen waarbij het minder goed gaat die we tegenkomen op een dienst kinderpsychiatrie. ”
En net over deze groep kinderen, de zogenaamde “klinische” groep adoptiekinderen, is nog niet zo veel bekend. “De meeste studies gaan niet specifiek over adoptiekinderen die naar een kinderpsycholoog of –psychiater werden verwezen. Deze klinische groep heeft een bijzonder profiel. We stelden bijvoorbeeld vast dat de kinderen die op onze dienst kwamen vaker moeilijkheden hadden op emotioneel, gedragsmatig en sociaal gebied. Ze deden het ook minder goed op school en behaalden lagere IQ scores dan een doorsnee populatie, wat deels te verklaren valt door hun taalachterstand. Het belangrijkste probleem was echter het gebrek aan een veilige hechting.”
Het begrip “hechting” werd ruim 50 jaar geleden geïntroduceerd door prof. John Bowlby, een Britse pionier in het toen nog jonge domein van de kinderpsychiatrie. Volgens zijn hechtingstheorie zoeken kinderen vanaf zeer jonge leeftijd de nabijheid van hun ouders op in situaties die stress veroorzaken. Ze doen dit zelfs nog vóór ze kunnen lopen, bijvoorbeeld door te huilen wanneer hun ouders de kamer verlaten. Naarmate ze opgroeien, worden ze gehecht aan zorgfiguren die betrouwbaar zijn en vormen ze zo een veilige basis, van waaruit ze kunnen exploreren. Oudere kinderen hebben minder nood aan de fysieke nabijheid van hun ouders, maar wel aan beschikbaarheid: ze willen weten dat ze op hun ouders kunnen rekenen wanneer dat nodig is.
Dr. Bonduelle licht verder toe: “De interactie tussen ouders en kinderen is bepalend voor de verwachtingen die kinderen ontwikkelen naar andere mensen toe. Kinderen die verwaarloosd of mishan-deld zijn, hebben dikwijls veel minder vertrouwen in latere relaties in hun leven. We weten dat deze traumatische ervaringen in de eerste levensmaanden en –jaren een risicofactor zijn voor het ontwikkelen van hechtingsproblemen, naast oudere leeftijd bij adoptie en een gebrek aan continuïteit in de verzorging van een kind. Al deze risicofactoren kwamen duidelijk vaker voor in onze klinische groep kinderen.”
De gevolgen van hechtingsproblemen zijn niet te onderschatten. Uit voorgaande studies blijkt dat veilig gehechte kinderen dikwijls beter scoren in verschillende andere ontwikkelingsdomeinen dan onveilig gehechte kinderen. Dit werd ook nagekeken in de studie in PAika. “We stelden vast dat kinderen met hechtingsstoornissen meer problemen ondervonden op emotioneel, gedragsmatig en sociaal gebied en een minder positief zelfbeeld hadden. Veilig gehechte kinderen deden het beter in deze domeinen. Er werd geen verband vastgesteld tussen intelligentie en hechting, zoals ook in eerder onderzoek beschreven werd.”
Ook de adoptieouders werden betrokken in de studie. Bij iedere adoptieouder werd o.a. gevraagd naar de band die ze hadden met hun eigen ouders tijdens hun jeugd, aan de hand van een vragenlijst. “Dit liet ons toe een idee te vormen over de opvoedingsstijl van hun ouders. Opvoedingsstijlen beïnvloeden de interactie tussen ouders en kinderen en dragen bij tot het beeld dat kinderen vormen over andere relaties, ook later in hun leven. Toch konden we in onze studie niet aantonen dat er een verband bestond tussen de opvoedingsstijl van de ouders van een adoptieouder en de band die de adoptieouders hadden met hun eigen kinderen.” Deze resultaten waren enigszins verbazend, aangezien in eerder onderzoek een vrij grote overeenkomst werd vastgesteld tussen de hechting van ouders en kinderen. Bij klinische populaties adoptiekinderen werd dit echter nog niet aangetoond.
De ouders vulden verder een vragenlijst in die peilde naar de moeilijkheden die ze ondervonden bij het opvoeden van hun kind. Daarbij gaven ze aan veel stress te ervaren, zeker bij kinderen met een hechtingsstoornis. “Onze resultaten benadrukken dan ook het belang van een brede, individueel aangepaste aanpak voor deze kinderen en van verdere opvolging van adoptiegezinnen na adoptie. Hoewel ze meestal uitgebreid voorbereid en gescreend werden voorafgaand aan de adoptie, gaven verschillende ouders aan dat ze weinig of geen begeleiding meer hadden gekregen na adoptie.”
