Ontwikkeling en evaluatie van een stappenplan voor een ergonomische interventie ter preventie van musculoskeletale aandoeningen

Preventie van musculoskeletale aandoeningen hangt grotendeels af van mindset werkgever.

Probleem?

“Men have become the tools of their tools.” (Thoreau, 1854, p. 29).

Eén zin beschrijft het opzet van deze eindverhandeling. Te lang is de mens gezien als een verlengde van zijn machines, waarbij hij zich diende aan te passen en te onderwerpen aan de gegeven arbeidsomstandigheden. Dat wreekt zich nu... Een groot aandeel van de Belgische populatie krijgt te kampen met musculoskeletale aandoeningen (MSA) (Eurofound, 2010). Dit zijn problemen, ongemakken, letsels, aandoeningen of zelfs beperkingen aan het bewegingsapparaat, veroorzaakt door de professionele activiteit (World Health Organisation, 2003; National Institute for Occupational Safety and Health, 2012; European Agency for Safety and Health at Work, 2007). Cijfers van de European working conditions survey voor de Belgische populatie laten dit duidelijk zien: 49,9% krijgt te maken met rugproblemen, 39,7% heeft spierpijnen in de bovenste ledematen en 27,3% in de onderste ledematen (Eurofound, 2010).

Deze cijfers zijn echter niet verwonderlijk gegeven dat men de dag van vandaag nog

steeds blootgesteld wordt aan fysieke risicofactoren (tillen & duwen van lasten, zwaar fysiek werk, ongunstige lichaamshoudingen, trillingen…) die MSA kunnen uitlokken (Eurofound, 2010, Nunens & Bush, 2012; Bevan, McGee & Quadrello, 2009). Daarnaast zijn er ook psychosociale (hoge jobeisen, lage jobcontrole, …) en individuele (BMI, roken … ) factoren die een rol spelen in het ontstaan. Het is echter moeilijk om de precieze oorzaak van MSA te achterhalen daar het een multicausaal en cumulatief probleem is. Er is wel bewijs dat werkgerelateerde risicofactoren weldegelijk effect hebben op de ontwikkeling en vorming van MSA (Punnet, 2014). Bovendien hebben MSA een grote impact op onze maatschappij in de vorm van medische kosten, langdurig ziekteverzuim, een verminderde levenskwaliteit, … (Bevan, McGee & Quadrello, 2009).

Preventie!Het preventief opsporen van de oorzaken en verminderen van risico’s kan er voor zorgen dat de kans op het verkrijgen van MSA aanzienlijk verkleind wordt (National Research Council [NRC], 1999; Van Nieuwenhuyse, Somville, Masschelein, Mairiaux & Moens, 2006). Effectiviteitsstudies laten bovendien zien dat deze risico’s kunnen aangepakt worden via interventies op de werkvloer en dat een multidimensionele aanpak loont. Tot op de dag van vandaag zijn we er echter nog niet in geslaagd om het MSA probleem op te lossen. Hoe komt dit?

Enerzijds komt dit omdat werkgevers andere prioriteiten stellen en anderzijds omdat er onvoldoende kennis is over de aanpak van dergelijke interventies (Van Peteghem et al., 2013).Er is dus nood aan een toegankelijke, uitgewerkte en systematische aanpak waar organisaties zelf mee aan de slag kunnen (Zalk, 2001). Vandaar dat er getracht werd een stappenplan te formuleren op basis van de literatuur dat tevens tegemoet komt aan de eisen van de praktijk. Dit plan werd tevens getoetst aan de praktijk, door 12 bedrijven/ervaringsdeskundigen te bevragen aan de hand van een semi-gestructureerd interview omtrent hun bevindingen over het stappenplan.

Wat kwam er uit?Het merendeel van de respondenten beschouwde het plan als bruikbaar en doeltreffend, echter minder haalbaar om het te gebruiken binnen de eigen organisatie. Ze schreven dit toe aan beperkende factoren, zoals het onvoldoende  bezitten van middelen en tijd, de grootte van de organisatie, de (overleg)cultuur binnen de organisatie, de sector waarbinnen men actief is etc. …  Deze opmerkingen met betrekking tot het plan waren echter dikwijls tegenstrijdig. Bepaalde factoren werden de ene keer als beperkend aangemerkt, en een andere keer weer als faciliterend. Dit kan verklaard worden door te wijzen op de verschillende ervaringen verbonden aan de eigen specifieke organisatie. Het valt bovendien sterk op dat er veel afhangt van de goodwill van werkgevers om deze beperkende factoren te counteren door het hanteren van een preventieve visie.

Preventie van MSA werkt pas voor honderd procent indien dit echt ingebouwd is in de organisatie, en het volledige stappenplan doorlopen wordt. De mindset hiervoor is bij vele werkgevers nog niet aanwezig zodat ze te veel curatief en te weinig preventief bezig zijn. Hierdoor zijn preventieadviseurs dikwijls genoodzaakt via achterpoortjes, lees gedeeltelijke maatregelen, binnen te sluipen in een bedrijf om dan nadien, stapsgewijs, de volledige preventie te kunnen doorvoeren.

Werk aan de winkelHoewel het inzicht in de MSA problematiek de laatste jaren sterk toegenomen is, en er hierdoor ook wettelijke maatregelen getroffen werden (denk aan de erkenning van bepaalde MSA aandoeningen als arbeidsziekten), kan er nog heel wat verbeterd worden. Er werd reeds een weg afgelegd, maar er moet meer komen. Niet alleen de bedrijven, maar ook de overheid moeten hier verder toe bijdragen.
