Bepaling van het aandeel vroedkundige activiteiten, gecoverd door VG-MZG, op materniteit en verloskunde
Vroedkundige werklast in tijden van een verkort materniteitsverblijf
De overheid heeft plannen om te besparen in de gezondheidszorg. Zo streeft ze naar een efficiënter gebruik van beschikbare ziekenhuisbudgetten. Deze plannen kunnen leiden tot een even veilig, maar korter materniteitsverblijf. De vraag kan gesteld worden of de vroedvrouw dan meer zorg zal verstrekken in kortere tijd. Hoe kan de werklast van de vroedvrouw er dan uitzien?
Om zicht te krijgen op de vroedkundige werklast is het nodig om de bovenstaande vraag in stukjes te verdelen. Wat wordt verstaan onder werklast? Welke activiteiten voert de vroedvrouw uit? Hoe kan vroedkundige werklast gemeten worden? Is de werklast dezelfde in alle ziekenhuizen? Hoeveel van de vroedkundige werklast is gekend door de overheid?
Om op deze vragen een antwoord te vinden, werd gespecialiseerde literatuur geraadpleegd. Bovendien werden 73 vroedvrouwen in vijf Vlaamse ziekenhuizen geobserveerd tijdens hun activiteiten op de afdelingen verloskunde en materniteit. Tussen de ziekenhuizen bevonden zich een universitair ziekenhuis en een ziekenhuis dat erkend is als baby vriendelijk ziekenhuis.
Wat wordt begrepen onder de term “werklast”? Werklast is de inspanning die nodig is om werk uit te voeren. Hierbij zijn de snelheid, de kwaliteit en de hoeveelheid van werk belangrijk. Vroedkundige werklast wordt hierbij onderverdeeld in patiëntenzorg en niet-patiëntenzorg.
Patiëntenzorg wordt uitgevoerd voor een specifieke patiënt.
Niet-patiëntenzorg bevat activiteiten die nodig zijn om de verpleegafdeling te doen draaien.
Wat doet de vroedvrouw? De vroedvrouw[1] is bevoegd voor de zorg rondom de geboorte. Hierbij kan ze medische zorgen verstrekken aan de moeder en de pasgeborene. Ze voert ook verpleegkundige taken uit. Hiertoe hoort de zorg voor basisbehoeften zoals voeding en hygiëne. De vroedvrouw dient geneesmiddelen toe en verzorgt wonden. Ze beperkt risico’s. Ze voorkomt ziektes en zorgt voor pijnstilling. Ze ondersteunt de moeder en de vader in hun rol als ouder. Hierbij geeft ze veel uitleg over allerlei onderwerpen zoals het babybadje en de voeding.
Op de ziekenhuisafdelingen verloskunde en materniteit komen bepaalde activiteiten veel voor. Naast uitleg geven, gerust stellen en overleggen, verzorgt de vroedvrouw administratieve en vroedkundige taken. Ze volgt veel parameters op, zoals de bloeddruk van de moeder. Ook het gewicht en de temperatuur van de baby volgt ze op. De vroedvrouw helpt de barende vrouw om zich te ontspannen. Ze volgt het verloop van de arbeid en de bevalling nauwgezet op. Na de bevalling ondersteunt ze de evolutie van de moeder en de baby. Ze moedigt het knuffelen met de baby aan. Ook zorgt ze ervoor dat moeder, vader en hun baby zoveel mogelijk samen kunnen blijven.
Hoe kan de werklast van vroedvrouwen gemeten worden? In België worden de vroedkundige activiteiten geregistreerd met het VG-MZG-meetinstrument[2]. Hieronder valt ook de vroedkundige werklast. De verkregen resultaten schetsen een betrouwbaar beeld van de zorgactiviteiten in de ziekenhuizen.
Is de werklast van vroedvrouwen overal dezelfde? Ja, ongeveer. In de huidige studie kan er geen betrouwbaar verschil in vroedkundige werklast vastgesteld worden tussen de verschillende ziekenhuizen en de verschillende beleidsvormen. De vroedkundige werklast in het universitair ziekenhuis, of in het baby vriendelijk ziekenhuis, verschillen niet van de werklast in de andere ziekenhuizen in de studie.
Welk aandeel van de vroedkundige werklast is gekend door de overheid? Ongeveer de helft van de vroedkundige werklast (49%) is gekend door de overheid.
Uit de studie blijkt dat de vroedvrouw 66% van haar arbeidstijd besteedt aan patiëntenzorg. Hiervan wordt 25% niet meegedeeld aan de overheid. Deze niet meegedeelde activiteiten hebben hun waarde in de praktijk niet bewezen. Daarnaast besteedt de vroedvrouw 34% van haar tijd aan niet-patiëntenzorg. Deze niet-patiëntenzorg wordt helemaal niet gerapporteerd.
Deze cijfers liggen in de lijn van de verpleegkundige werklast op andere ziekenhuisafdelingen. De gegevens in deze studie zijn betrouwbaar. Toch ligt de betrouwbaarheid van de gegevens hoger op de afdeling materniteit dan op de afdeling verloskunde. Tevens zijn de gegevens van de activiteiten die uitgevoerd worden bij de moeder betrouwbaarder dan bij de pasgeborene. Vermoedelijk is het verschil in grootte van de onderzochte groepen hiervan de oorzaak.
Wat is nu het praktisch nut van deze studie?
Er wordt een helder beeld gegeven van de activiteiten van de vroedvrouwen. In het merendeel van de dagelijkse activiteiten van de vroedvrouw is er sprake van patiëntenzorg. Van de vroedkundige activiteiten blijkt ongeveer de helft een bewezen waarde te hebben. Het kan nuttig zijn om na te gaan welke activiteiten zonder bewezen waarde kunnen verminderd of vermeden worden. Zo verplaatsen de vroedvrouwen zich veel in de grote ziekenhuizen. Deze verplaatsingen kunnen bijvoorbeeld verminderd worden door een efficiënte inrichting van een verpleegafdeling of een zorgvuldig opgemaakte zorgplanning.
De onvoorspelbaarheid van vroedkundige werklast bemoeilijkt een correcte personeelstoewijzing. Door rekening te houden met de periodes waarin de meeste spontane geboortes plaatsvinden is het mogelijk om een gepast aantal personeelsleden te voorzien.
In de studie worden verschillende werklast-meetinstrumenten vergeleken. Zo houdt het Birthrate Plus® instrument rekening met de afhankelijkheid van de zwangere en de moeder-kind eenheid. Het Belgische VG-MZG-instrument houdt rekening met de activiteiten bij moeder én kind. Uit deze vergelijking blijkt dat Belgische vroedkundige werklast het best gemeten wordt met het VG-MZG-instrument. Dit instrument voldoet, zelfs al wordt slechts de helft van de activiteiten gerapporteerd.
Vanuit de studie kan tegemoet gekomen worden aan de bezuinigingsplannen van de overheid. Zo kan het interessant zijn om het meest rendabel zorgprofiel op te stellen. Tevens kan nagegaan worden hoe het aandeel ‘niet-patiëntenzorg’ kan verlaagd worden. Er kan onderzocht worden of een geboortecoach kan instaan voor continue ondersteuning van de zwangere tijdens de arbeid. Deze niet medisch geschoolde coach, of doula, kan ook thuis verdere ondersteuning bieden.
De invoering van georganiseerd multidisciplinair overleg kan een positieve invloed hebben op de vroedkundige werklast en op efficiëntie. Dit overleg tussen gynaecologen, pediaters, vroedvrouwen, kinesisten, en eventuele thuiszorg, zou de communicatie ten goede kunnen komen. Communicatie blijkt nu net de meest uitgevoerde activiteit van de vroedvrouwen te zijn. Is deze studie niet hoopgevend?
