Breast cancer related lymphedema prior to and 36 months following adjuvant radiotherapy: a randomised controlled trial comparing conventional radiotherapy and short-course tomotherapy

Invloed van bestralingstechniek bij borstkanker op het ontstaan van lymfoedeem

Volgens de recentste statistieken uit 2012 van de Stichting Kankerregister werden in België maar liefst 10.531 nieuwe gevallen van borstkanker geregistreerd. Borstkanker is algemeen gekend als de meest voorkomende kanker bij vrouwen. Sterker nog, een kwart van de borstkankerpatiënten overleeft de ziekte niet.

De behandeling van borstkanker bestaat meestal uit een combinatie van heelkunde, bestraling, chemotherapie of immunotherapie (deze laatste maakt gebruik van antilichamen). Wereldwijd onderzoek naar borstkanker richt zich enerzijds op het in kaart brengen van de ontstaansmechanismen, anderzijds op de ontwikkeling van nieuwe of verbeterde behandelingen. Binnen de bestraling is er een evolutie naar kortere en meer nauwkeurig gerichte bestraling. Immers, bestralingstherapie bij borstkanker doet de overlevingskansen stijgen, maar  kan tegelijkertijd ook leiden tot fysieke beperkingen, zoals verminderde schoudermobiliteit en lymfoedeem in de arm langs de zijde van de aangetaste borst. Tomotherapie is een innovatieve bestralingstechniek die de bestraling nauwkeuriger weet toe te dienen door meerdere technieken met elkaar te combineren. Met deze techniek ontstond de mogelijkheid om de dagelijkse bestralingshouding nauwgezet te controleren.

Het UZ-Brussel voerde een studie uit bij borstkankerpatiënten, waarbij Tomotherapie werd vergeleken met een klassieke bestralingstechniek. Enkel patiënten die heelkunde ondergingen voor hun borstkanker en die geen of beperkte klieraantasting hadden, namen deel aan de studie. De primaire eindpunten van deze studie betroffen de neveneffecten op longen en hart. In tweede tijd werden ook de fysieke beperkingen bekeken. 123 patiënten werden geselecteerd om deel te nemen. Zij werden verdeeld in twee groepen, rekening houdend met het stadium van de kanker, het type heelkunde dat ze ondergingen en of zij chemotherapie kregen. De ene groep kreeg Tomotherapie toegediend, de andere klassieke bestralingstherapie.

Dit onderzoek was onderdeel van deze grotere studie en richtte zich op één van de mogelijke fysieke beperkingen van de borstbehandeling, namelijk het ontstaan van lymfoedeem van de arm. Lymfoedeem kan ontstaan als gevolg van okselheelkunde, van chemotherapie of van bestraling van de oksel. Het meest risico bestaat bij de combinatie van deze drie behandelingen.

Voor de start en op verschillende momenten na het beëindigen van de bestraling werd het volume van beide armen berekend via omtrekmetingen. Hieruit werd het absoluut en relatief volume verschil tussen de beide armen afgeleid. Dit gaf aan of er sprake was van lymfoedeem. Ook werd er door ondervraging gepolst naar de aanwezigheid van bepaalde symptomen die typerend zijn voor lymfoedeem, zoals tintelingen en een gespannen gevoel in de arm.

Drie jaar na de behandeling vonden we geen betekenisvol verschil tussen beide bestralingstechnieken in de ontwikkeling van lymfoedeem. Het eerste jaar na de bestraling zagen we zowel na Tomotherapie als na klassieke bestralingstherapie het volume in de arm aan de aangetaste zijde geleidelijk stijgen. Dit bevestigde de resultaten van voorgaande studies. Na deze periode zagen we dit volume opnieuw afnemen tot 3 jaar na de behandeling.

Verder onderzoek naar de invloed van kinesitherapeutische behandelingen en de invloed van mogelijke aandoeningen die lymfoedeem in de hand werken, is noodzakelijk. Intussen kijken we alvast uit naar de resultaten van de primaire eindpunten van de overkoepelende studie, namelijk de neveneffecten op longen en hart. 

Verklarende begrippen:

Lymfoedeem duidt op een ongewenste opstapeling van lymfevocht.  In normale omstandigheden nemen lymfevaten het lymfevocht op, waarna de lymfeklieren het lymfevocht zuiveren. Tot slot brengen de lymfevaten het vocht terug naar de bloedvaten. Wanneer de lymfevaten het lymfevocht niet kunnen opnemen en afvoeren, ontstaat er lymfoedeem.

Tomotherapie is een bestralingstoestel waarbij de bestraling vanuit een volledige cirkel worden toegediend terwijl de persoon door de cirkel schuift. Het combineert een nauwkeurig gerichte bestraling met dagelijkse beeldvorming om de exacte bestralingshouding te controleren.
