De biopsychosociale aanpak van niet-cardiale borstpijn op een spoedgevallendienst

Pijn op de borst: altijd ernstig, toch?   

Joost is 22, voelt zich goed in zijn vel, sport veel en probeert gezond te eten. Af en toe ervaart hij echter een hevige stekende pijn op de borst. Hij voelt dan hoe zijn hart bonkt in zijn keel en het lijkt alsof ademen hem tien keer zo veel moeite kost. Meestal gaat het snel over maar de laatste keer duurde het zo lang dat hij op spoed terecht kwam. De dokters verzekerden hem dat zijn hart kerngezond was maar zoals altijd was Joost er nog niet helemaal gerust in…

Velen ervaren het verhaal van Joost als herkenbaar. Als je het al hebt meegemaakt, zul je kunnen bevestigen dat het een nare en beangstigende ervaring is die voor onzekerheid zorgt. In de medische wereld wordt dit OBP (Onverklaarde Borstpijn) genoemd. Zij die het nog niet meemaakten hebben een grote kans om het alsnog te ondervinden: een vierde van de bevolking krijgt er wel eens mee te maken.

“Het komt wel goed”

Niettegenstaande de goede resultaten van onderzoeken, geven de patiënten met OBP vaak aan zich echt ziek te voelen, ongerust te zijn en zich niet zeker te voelen door de meestal vage aanbevelingen en opvolging. Bij sommige patiënten wordt er een onderliggende aandoening gevonden (zoals een klaplong, hyperventilatie of middelenmisbruik) maar meer dan 50 % van de patiënten op spoed met hartklachten of pijn op de borst krijgen geen specifieke diagnose. Dit zijn de OBP-patiënten.

Het is door de onzekerheid waarin zij zich bevinden dat er al snel een vicieuze cirkel kan ontstaan. Angstige patiënten kennen een aandachtsverschuiving in de richting van hun gewaarwordingen (pijn, hartkloppingen,… ) waardoor ze zich hier onbewust meer op gaan focussen. Deze focus doet de frequentie toenemen waarmee ze bepaalde dingen gaan voelen en zorgt dus op zijn beurt voor meer angst.

Patiënten met OBP mogen dan wel lichamelijk volledig gezond zijn, het alom bekende zinnetje ‘het komt wel goed, u mankeert niets’ is voor hen weinig hulpzaam.

Paniek

Betekent dit dan dat OBP nooit ernstig kan zijn? Dat is niet het geval, integendeel: studies wijzen uit dat er heel wat psychologische factoren gekoppeld zijn aan de problematiek van OBP. Zo blijkt 88 % van alle patiënten met onverklaarde borstpijn te lijden aan een onderliggende psychiatrische aandoening, waarvan paniekstoornissen de meest frequent voorkomende zijn. Opmerkelijk is wel dat slechts 6 % van die paniekstoornissen correct geïdentificeerd wordt op een spoedgevallendienst. Met andere woorden: 94 % blijft onder de radar.

Zonder een juiste en gerichte aanpak van zo’n stoornis dient de patiënt terug te vallen op diens eigen strategieën in een poging opnieuw controle te krijgen over angstgevoelens en lichamelijke sensaties. Deze strategieën kunnen echter zeer schadelijk zijn voor het individu. Het gaat bijvoorbeeld over het vermijden van bepaalde activiteiten, plaatsen (waar men eerder een aanval kreeg) of personen. Zo’n gedrag heeft een nefast effect op de sociale en mentale gezondheid van de patiënt en kan net zo hinderend zijn als een lichamelijke ziekte. ‘Gewoon paniek’ is dus geen goede stelling.

Bovendien heeft OBP de neiging om gemakkelijk naar een chronische vorm te evolueren. Heel wat patiënten ondervinden jaren na de eerste klachten nog dagelijks moeilijkheden.  

Economische impact

Niet alleen de patiënt maar ook het volledige gezondheidszorgsysteem heeft baat bij een goede opsporing van de onderliggende aandoeningen van onverklaarde borstpijn. Aangezien OBP-patiënten vaak beroep doen op gespecialiseerde gezondheidszorg (zoals een spoeddienst, cardiologen, …) kunnen de kosten per patiënt flink oplopen. Een studie uit 2013 schatte de jaarlijkse kosten per OBP-patiënt op € 10 068.

Niet elk verhaal van OBP is even extreem maar elk verhaal verdient de nodige aandacht en zorg. Op die manier worden bepaalde vicieuze cirkels tijdig doorbroken, spaart de gezondheidszorg een significante kost uit en kan Joost blijven genieten van hetgeen hij het liefste doet.
