N-Acetylcysteïne: een nieuw geneesmiddel voor de dodelijke bloedziekte trombotische trombocytopenische purpura?

In 1924 stierf een 16-jarig Amerikaans meisje na het plotse onstaan van bloeduitstortingen, bloedarmoede, koorts, verlamming en uiteindelijk ook coma. Dit was het eerste geval van de bloedziekte trombotische trombocytopenische purpura (TTP): een hele mond vol voor een uiterst zeldzame en levensbedreigende ziekte. Momenteel komt TTP bij zo’n vier miljoen personen per jaar voor. Zonder behandeling is TTP meestal fataal. De huidige therapieën voor deze ziekte kennen echter veel nadelen. Daarom moet er zo snel mogelijk een alternatief voor deze patiënten worden gevonden. Het geneesmiddel N-Acetylcysteïne werd tijdens dit onderzoek getest als nieuwe behandeling voor TTP.

Trombotische trombocytowat??

“Trombotische”: Bij TTP worden er kleine trombi gevormd in ons bloed. Deze ontstaan doordat het eiwit von Willebrand Factor (VWF), dat belangrijk is voor het stelpen van bloedingen, onvoldoende wordt geknipt in ons bloed. Hierdoor circuleren er te lange ketens van deze factor doorheen de bloedbaan. Aan deze ketens binden ook veel bloedplaatjes. De vorming van een netwerk van bloedplaatjes en VWF kan zo’n trombus vormen. Dit kan kleine bloedvaten afsluiten, waardoor de zuurstoftoevoer naar bepaalde organen beperkt wordt. Zonder intensieve behandeling leidt dit zuurstoftekort tot de dood van de meeste TTP-patiënten.

“Trombocytopenische”: De ziekte wordt ook gekenmerkt door een tekort aan vrije bloedplaatjes in de bloedbaan, aangezien bloedplaatjes gevangen zitten in de trombi. Dit tekort wordt ook trombocytopenie genoemd.

“Purpura”: TTP-patiënten vertonen bovendien vaak rode, puntvormige stippen op de huid, ook pupura genoemd. Deze bloeduitstortingen onstaan doordat kleine bloedingen, die ook bij gezonde personen voorkomen, niet meer kunnen worden gestopt door het lage vrije bloedplaatjesaantal bij TTP.

De naam trombotische trombocytopenische purpura weerspiegelt dus de drie belangrijke symptomen van deze ziekte.

Huidige behandelingen voor TTP schieten vaak tekort

De belangrijkste huidige behandeling voor TTP bestaat uit het vervangen van het bloedplasma. Hierbij wordt het plasma van de patiënt, dat een tekort heeft aan een functionele knipfactor voor VWF, vervangen door het plasma van een donor. Dit donorplasma bezit de factor wel voldoende. Deze vorm van therapie heeft echter heel wat nadelen: ze is duur, tijdrovend, vermoeiend en gevaarlijk. Er moet dus dringend een nieuwe behandeling voor deze zeldzame ziekte gevonden worden.

N-Acetylcysteïne: een alternatief voor een therapie vol nadelen?  

N-Acetylcysteïne, afgekort NAC, is een geneesmiddel dat gebruikt wordt bij luchtwegeninfecties en overdosissen paracetamol. In het labo werd reeds aangetoond dat NAC de factor VWF kan knippen. Helaas strookt de situatie in de proefbuis niet altijd met de werkelijkheid. Het geneesmiddel moest dus ook geanalyseerd worden bij dieren. Tijdens dit onderzoek ging men na of NAC werkzaam is bij muizen met de ziekte TTP. Het resultaat bleek negatief: NAC is niet effectief in het verwijderen van kleine trombi.

Opmerkelijk was dat NAC wel werkzaam was ter preventie van TTP in muizen. Dit is een belangrijk fenomeen, aangezien TTP-patiënten vaak hervallen. Omdat er niet veel nadelen gekend zijn bij het gebruik van NAC op lange termijn, zou dit geneesmiddel een goede oplossing kunnen bieden voor deze hervallende patiënten.

NAC is effectief ter preventie, maar niet ter genezing van TTP in muizen

Hoewel bloedplasma-uitwisseling al veel levens heeft kunnen redden, heeft de behandeling veel nadelen en is men dringend op zoek naar een alternatief. NAC is al jaren verkrijgbaar als slijmoplossend middel of ter behandeling van paracetamol overdossisen. Omdat NAC onder laboratoriumcondities de factor VWF kan knippen, wat niet meer mogelijk is in geval van TTP, werd het verder getest op muizen. NAC kon muizen met TTP niet genezen, maar het was wel werkzaam ter preventie van TTP. Daarom zou NAC mogelijk kunnen worden gebruikt om het herval van patiënten met TTP te vermijden. Ondertussen is dit werk onder revisie bij het toonaangevende wetenschappelijke tijdschrift “Blood” en worden er TTP patiënten gerecruteerd om deel te nemen aan een klinische studie met dit geneesmiddel!
