Blast Injuries: prehospitale benadering

Terreuraanslagen komen vaak voor in de vorm van bomaanslagen en gebeuren meestal op populaire, openbare plaatsen. Denk maar aan de aanslagen in Brussel op 22 maart
2016. Door de snelle toename van terrorisme in de Westerse landen en het stijgende gebruik van explosieven, is het van belang dat zorgverleners vertrouwd raken met het management van de verwondingen die het gevolg zijn van een explosie. Deze bachelorproef zal trachten een antwoord te bieden op de vraag ‘Wat zijn de verpleegkundige aandachtspunten bij blast injuries na een explosie?’ Het doel van deze bachelorproef is dan ook om de zorg voor slachtoffers met blast injuries te kunnen
optimaliseren.

Om dit project tot stand te brengen werd er een literatuurstudie uitgevoerd over een periode van oktober 2016 tot mei 2017. Verder werden er nog enkele bijscholingen en
congressen bijgewoond die relevant zijn aan dit onderwerp. Voor het bepalen van de inhoud van deze bachelorproef zijn er ook medische experten geraadpleegd. In het algemeen gedeelte van deze bachelorproef wordt er dieper ingegaan op het ontstaansmechanisme van een explosie, de specifieke verwondingen die deze impact met zich meebrengt en de veiligheid die hulpverleners in acht moeten nemen. In het specifiek gedeelte ligt de focus op drie verschillende onderwerpen: damage c ontrol, triage en blast
injuries bij het kind.

Blast injuries zijn gecompliceerde verwondingen die ontstaan door de verschillende mechanismen die plaatsvinden bij een explosie. De verwondingen worden ingedeeld in
verschillende categorieën: primaire, secundaire, tertiaire, quaternaire en quinaire verwondingen. Het mechanisme dat aan de oorzaak ligt van de blast injuries en het
letseltype bepaald in welke categorie de wonde wordt ingedeeld.

Door de vernietigende kracht van een explosie kan de omgeving heel onoverzichtelijk zijn, waardoor het moeilijk is om de veiligheid van hulpverleners te garanderen. Er zijn
steeds verschillende risico’s (instortingsgevaar, secundaire explosieven, toxische stoffen, …) die niet op voorhand kunnen uitgesloten worden. Hierdoor kan de veiligheid van de
hulpverleners nooit volledig gegarandeerd worden. De nodige aandacht voor veiligheidsmaatregelen en een multidiscpiplinaire samenwerking met andere disciplines
kan de risico’s aanzienlijk verminderen.

Het specifieke karakter van blast injuries maakt dat de opvang van de slachtoffers anders zal gebeuren dan in een klassieke benadering. Het concept ‘Damage Control’ toont het
belang van een snelle eerste hulpverlening. De prehospitale triage bij een explosie verloopt niet zoals een standaard triage omwille van het grote aantal slachtoffers en de
onoverzichtelijke situatie. Kinderen kunnen eveneens het slachtoffer worden van een explosie. Op meerdere
domeinen kunnen verschillen ten opzichte van volwassenen vastgesteld worden. Het onderscheid uit zich zowel op anatomisch, fysiologisch, immunologisch, ontwikkelings-,
en psychologisch vlak. Deze verschillen beïnvloeden de kwetsbaarheid voor het ontwikkelen van letsels en de manier van reageren op een ramp. Wanneer er bij de
triage, de diagnose en de behandeling geen rekening gehouden wordt met deze verschillen kan dit het risico op ernstige restletsels of overlijden verhogen.

Uit de gevonden literatuur blijkt dat er nog onvoldoende en/of tegenstrijdige informatie beschikbaar is over bepaalde onderwerpen. Deze zijn onder andere het gebruik van
hemostatisch verband, prehospitale triage en de specifieke behandeling van blast injuries bij kinderen.
