De psychiatrie slaat de bal mis

Amber is 14 als haar moeder zich van het leven berooft. Haar vader krijgt niet veel later een drankprobleem en plots staat Amber er alleen voor. Ze moet niet enkel voor haar vader, maar ook voor haar twee jongere broertjes zorgen. Ze verwaarloost haar schoolwerk en krijgt het huishouden maar moeilijk rond. Als Amber 18 wordt, begint ze een stem te horen.

Het is de stem van Ambers moeder, die haar kleineert en telkens opnieuw zegt dat ze er beter een einde aan zou maken. Amber begint zich af te zonderen en komt tot helemaal niets meer. Tot ze gedwongen wordt opgenomen in de psychiatrie. Ze krijgt verschillende pillen voorgeschreven, die er maar niet voor zorgen dat de stem weggaat. Integendeel, ze hoort nu ook andere stemmen die ervoor zorgen dat ze helemaal niet meer kan functioneren. Ze raakt in een vicieuze cirkel van opnames terecht. Al na enkele maanden volgt het verdict van de psychiater: Amber lijdt aan schizofrenie.

What's in a name?

Het fictieve verhaal van Amber is geen geval op zich. Zowat iedereen komt gedurende het leven in contact met de psychiatrie. Volgens Geestelijk Gezond Vlaanderen heeft één op vier mensen vroeg of laat af te rekenen met ernstige psychische problemen. Een belangrijke en onbegrepen groep mensen met een psychische kwetsbaarheid krijgt een psychose. Een kortdurende psychotische episode kan volgens de visie van de westerse psychiatrie resulteren in schizofrenie, een vaak gediagnosticeerde, ernstige en ongeneesbare psychotische aandoening.

Het is niet eenvoudig om weer te geven wat een psychose, laat staan schizofrenie, precies inhoudt. In feite kan er worden gesteld dat er volgens psychiaters minstens één van twee symptomen aanwezig moet zijn om te kunnen spreken van een psychotische aandoening: waangedachten en/of hallucinaties. Zo is er in de casus van Amber sprake van auditieve hallucinaties: ze hoort stemmen die anderen niet horen.

De benadering van iemand die een psychose beleeft, is niet vanzelfsprekend. Ondanks 100 jaar ononderbroken inzet om psychose beter te begrijpen, slaat de psychiatrie de bal mis. Mensen die een opname kenden omwille van een psychotische kwetsbaarheid verklaren vaak dat ze niet begrepen werden en dat geneesmiddelen werden opgedrongen. Ze vergelijken hun opname met gevangenisopnames of martelpraktijken en raken in herval.

Het loopt al jaren mis

De psychiatrie baseert zich al decennialang op het stellen van een diagnose om van daaruit symptomen te gaan behandelen. Deze diagnoses baseren zich op een handboek dat de Diagnostic and Statistical Manual of Mental Disorders (DSM) wordt genoemd. Het probleem is dat dit handboek niet gebaseerd is op wetenschappelijk onderzoek. Het is eerder een verzameling van wat psychiaters observeerden bij mensen met psychische problemen. Zo trachtten ze een onderscheid te maken tussen verschillende soorten van mentale verstoring.

De kritiek die geuit wordt op het gebruik van de DSM neemt met de jaren toe. Onder meer Allen Frances, die ooit betrokken was bij de totstandkoming van het handboek, richt zich nu tegen het gebruik ervan. Ook Jim Van Os, Nederlands Hoogleraar in de Psychiatrie en voortrekker van de website Psychosenet, geeft aan dat diagnosestelling van psychotische stoornissen bijdraagt aan de inadequate behandeling van deze problematiek.

Het idee dat waangedachten en hallucinaties ontegensprekelijk gekoppeld zijn aan een psychiatrische problematiek is volledig uit de lucht gegrepen. 90% van de gerapporteerde psychotische ervaringen worden niet in verband gebracht met een psychiatrische aandoening. Zo bestaan er bijvoorbeeld culturen waar mensen die entiteiten channelen een hoog aanzien wordt toegekend. Of er sprake is van ziekte is bijgevolg enkel afhankelijk van het wereldbeeld van de hulpverlener. Van Os durft zelfs te stellen dat we allemaal psychotische ervaringen hebben. We hebben namelijk allemaal wel eens het gevoel achtervolgd of bekeken te worden tijdens een nachtwandeling in de stad terwijl dit niet het geval is.

Daarom wordt er steeds meer geopperd om de term 'psychotische symptomen' achterwege te laten en te erkennen dat 'atypische ervaringen' heel eigen zijn aan het menselijk bestaan.

Uiteraard is er bij de doelgroep die hulp nodig heeft binnen de psychiatrie sprake van ernstigere realiteitsvervormingen die nefast zijn voor het kunnen functioneren in de samenleving. Hoewel zij deze externe hulp duidelijk nodig heeft, schiet de psychiatrie tekort. De ellenlange zoektocht naar een evenwicht tussen opklaring van symptomen en bijwerkingen van antipsychotische medicatie werkt demotiverend en brengt een gedaalde levenskwaliteit met zich mee. Medicatie schenkt bovendien vaak slechts een tijdelijke verlichting van de ervaringen en gaat niet naar de kern van het probleem. Het moet anders en het moet beter.

Nood aan ruimte voor herstel

De psychiatrie zou bijgevolg eerder een plaats moeten zijn waar er erkenning wordt gegeven aan het feit dat een crisis een natuurlijk gevolg is van onverwerkt emotioneel trauma. Een plaats waar men kwetsbaar mag zijn en men niet zo snel als mogelijk terug buiten moet. Herstel is een proces dat ruimte nodig heeft.

Er zijn tal van alternatieven die hun effectiviteit bewezen hebben in dit herstelproces voor mensen met een psychotische kwetsbaarheid. Momenteel worden deze in de psychiatrie echter nauwelijks aangehaald.

Vooreerst moet er een rustige en veilige ruimte gecreëerd worden voor mensen die een bestaanscrisis ervaren. Hulpverleners dienen veeleer beschikbaar te zijn dan dat ze therapieën opdringen. Toch dient een breed scala aan therapeutische mogelijkheden voorhanden te zijn. Zo kunnen hulpverleners en cliënten samen op zoek gaan naar een efficiënte behandeling. Bovendien creëren hulpverleners op die manier hoop, wat in schril contrast staat met het toekennen van een ongeneesbare aandoening. Enkel zo kan de huidige hiërarchische beleving van cliënten naar de achtergrond verdwijnen.

Er is met andere woorden een dringende nood aan meer menselijkheid in de zorg. Op die manier kan er eindelijk afgestapt worden van de behandeling van 'een schizofreen', naar het op stap gaan met een mens.

Misschien had ook Amber op die manier een plaats gevonden waar ze haar kwetsbaarheid had leren erkennen. Een plaats waar ze haar traumatische ervaringen had kunnen verwerken en had aangeleerd hoe ze met de stemmen die ze hoorde om kon gaan. Een plaats waar ze zich eindelijk opnieuw thuis had kunnen voelen.
