Zwanger na een fertiliteitstraject - Impact op de beleving van zwangerschap, postpartum en pril ouderschap

‘Normaal’ maar toch ‘niet normaal’: zwanger na IVF

Zwanger worden. Het lijkt de meest natuurlijke zaak van de wereld, maar dat is het lang niet altijd. Het Studiecentrum voor Perinatale Epidemiologie becijferde dat in het jaar 2015 alleen al 4448 zwangerschappen in Vlaanderen het gevolg waren van een behandeling voor onvruchtbaarheid. Ingrepen zoals hormonale stimulatie, kunstmatige inseminatie en vooral in-vitrofertilisatie (IVF), al dan niet in combinatie met intracytoplasmatische sperma-injectie (ICSI) waarbij de zaadcel in de eicel wordt gebracht, zijn de laatste 25 jaar in opmars. Het is evident dat het realiseren van een zwangerschap het eerste streefdoel is van de fertiliteits-geneeskunde, maar wat daarna? Hoe beleef je als vrouw en als koppel een zwangerschap die niet op een natuurlijke manier ontstond?

 

Tot op vandaag worden vrouwen na een kunstmatige bevruchting op dezelfde manier opgevolgd als vrouwen met een natuurlijke zwangerschap. Toch kunnen we ons de vraag stellen of dat wel helemaal terecht is. Je zíet natuurlijk geen verschil tussen deze vrouwen, maar zowel fysiek als mentaal is extra begeleiding bij IVF-zwangerschappen geen overbodige luxe. In veel gevallen is de zwangerschap het resultaat van een lange en emotionele weg, waarvan de psychologische belasting zwaar is. Ook het sociale stigma dat verbonden is aan onvruchtbaarheid is voor veel koppels niet makkelijk los te laten, zélfs wanneer ze uiteindelijk zwanger worden. Naast de psychosociale kwetsbaarheden zijn er ook risico’s verbonden aan de zwangerschap en bevalling op zich. De kans op een vroeg miskraam en een buitenbaarmoederlijke zwangerschap is groter, net zoals het risico op placentaproblemen, zwangerschapsdiabetes, hoge bloeddruk en zwangerschapsvergiftiging. In het geval van ICSI zou er bijkomend een grotere kans bestaan op aangeboren afwijkingen bij de baby. Het is daarom niet ondenkbaar dat vrouwen en koppels die op een kunstmatige manier zwanger werden, andere – of bijkomende – noden en verwachtingen hebben in vergelijking met een spontane zwangerschap.

 

Dichten van de kloof

De voorbije jaren is er geleidelijk aan meer aandacht gekomen voor de manier waarop vrouwen en koppels met vruchtbaarheidsproblemen een IVF-zwangerschap ervaren. Zo is het voor deze koppels vooral belangrijk dat ze tijdens de zwangerschap regelmatige begeleiding op maat krijgen. We mogen niet vergeten dat ze in veel gevallen een intensieve en emotioneel belastende periode in de fertiliteitskliniek hebben doorgemaakt. Enerzijds zijn ze blij dat deze fase achter de rug is en dat hun zwangerschap wordt opgevolgd ‘zoals alle andere’. Maar aan de andere kant blijven ze een grotere behoefte hebben aan controles en aan aandacht voor hun uitzonderlijke situatie. Ze voelen zich normaal, maar toch weer niet zo normaal.

Door die tegenstrijdige gevoelens ervaren veel van deze vrouwen, en vaak ook hun partners, in verschillende opzichten een kloof bij de verdere opvolging van hun zwangerschap. Eerst en vooral voelen ze een tijdskloof tijdens de weken die voorbij gaan tussen de laatste controle in de fertiliteitskliniek en de eerste afspraak bij de vroedvrouw of gynaecoloog die de zwangerschap verder opvolgt. Voor koppels die een (lang) traject hebben doorstaan om zwanger te worden, is dit vanzelfsprekend een periode van grote onzekerheid en bang afwachten. Ten tweede kunnen vrouwen na het verlaten van het fertiliteitscentrum met een kenniskloof geconfronteerd worden. Ze vinden dat vroedvrouwen en artsen niet goed op de hoogte zijn van hun voorgeschiedenis – of dat ze dit in ieder geval te weinig laten merken. Tot slot ervaren vrouwen met een IVF-zwangerschap in veel gevallen een zorgkloof. De angst of twijfel waar vrouwen mee te maken krijgen tijdens de eerste helft van de zwangerschap, is bij vrouwen die zwanger werden via IVF vaak nog een stuk groter en overheersender. Het standaard opvolgschema – met controles om de 4 tot 6 weken – schiet in veel gevallen tekort om aan hun onzekerheid tegemoet te komen. 

Het ‘dichten van de kloof’ is daarom iets waar naar gestreefd moet worden, en net daar kan de vroedvrouw een grote rol spelen. Een vroege eerste afspraak aanbieden zal de tijdskloof beperken en de overgang draaglijker maken. Expliciet vragen naar de manier waarop hun vroegere ervaringen de beleving van de zwangerschap beïnvloeden, kan zorgen voor een gevoel van begrip en opent de weg naar het uiten van emoties. Tot slot zouden vroedvrouwen na moeten gaan of de vrouwen behoefte hebben aan frequentere afspraken om hun zwangerschap te bevestigen en bezorgdheden te bespreken.

 

Ruimte voor negatieve gedachten

Maar niet alleen de andere noden naar zorgverlening toe maken een zwangerschap na kunstmatige bevruchting anders. IVF behandelingen zijn duur en worden in België tot 6 keer terugbetaald door het ziekenfonds. Hierdoor is onbewust de opvatting ontstaan dat koppels die zwanger worden via zo’n dure behandeling in de eerste plaats dankbaar en gelukkig moeten zijn. Onze maatschappij biedt geen ruimte voor negatieve gevoelens en gedachten die het gevolg kunnen zijn van het behandelingstraject, van zwangerschap op zich, of door het besef van het aankomende ouderschap. Er wordt onterecht vanuit gegaan dat deze toekomstige ouders immuun zijn voor negatieve gevoelens omdat het succes van de zwangerschap dit alles overstijgt. Vrouwen en koppels die zwanger werden via IVF vervallen hierdoor makkelijk in self silencing: ze verzwijgen hun échte gevoelens en gedachten of gaan deze overdreven relativeren, omdat ze ervan uitgaan dat deze sociaal niet gepast of aanvaardbaar zijn.

Ook hier kan de vroedvrouw het verschil maken. In de eerste plaats kan ze erop toezien dat onterechte gevoelens van schuld of overdreven dankbaarheid niet ontstaan of worden gestimuleerd. En als deze gedachten er toch zijn, kan de vroedvrouw ze bespreekbaar maken en doen inzien dat het niet abnormaal is om deze te hebben.  

 

Moeten vrouwen die zwanger werden na een vruchtbaarheidsbehandeling dan fundamenteel anders behandeld worden? Waarschijnlijk niet – al betekent dit ook niet dat ze automatisch mogen worden gelijkgesteld met vrouwen die spontaan zwanger werden. Zorgverleners, en op de eerste plaats vroedvrouwen, zouden zich op elk moment bewust moeten zijn van de impact die zo’n behandeling kan hebben op de zwangerschapservaring. Want een ‘gezonde zwangerschap’, niet louter fysiek maar in álle opzichten, moet uiteindelijk het streefdoel zijn van een vruchtbaarheidsbehandeling. Alleen zo creëren we niet alleen gezonde baby’s, maar bieden we tegelijk ook zorg op maat aan deze groeiende groep van toekomstige ouders.
