Attitudes van artsen en studenten genees- en verpleegkunde met betrekking tot gezamenlijke besluitvorming: een cross-sectionele studie

De patient als medebeslisser in zijn zorg: feit of fabel in Vlaanderen?

Inleiding

“It takes at least two to tango” is de titel van het wetenschappelijk artikel van Charles C. en Gafni A. (1997) dat velen auteurs beschouwen als een pionierswerk wanneer het gaat over gezamenlijke besluitvorming. Gezamenlijke besluitvorming houdt in dat de zorgverlener en de patiënt samen tot een besluit komen die gebaseerd is op de meest recente wetenschappelijke kennis én waarin ze beiden hun voorkeuren, kennis en waarden delen. Dit begrip heeft veel nationale bekendheid in de Verenigde Staten en Canada sinds 2001, maar ook Nederland, Duitsland en Frankrijk hebben dit recent opgepikt. Het wordt steeds meer door beleidsmakers en vertegenwoordigers van professionele en patiëntenorganisaties aangeraden om deze vorm van besluitvorming de norm te maken in de gezondheidszorg.

Vanzelfsprekend of utopie?

Hoe zit het met de realiteit?

Het lijkt op eerste zicht vanzelfsprekend dat artsen rekening houden met de persoonlijke waarden, kennis en voorkeuren van de patiënt. Een groot aantal studies toont echter het tegendeel aan, zo zou maar 10% van alle gesprekken met de arts volgens dit principe verlopen. De hoofdredenen hierachter zijn onder andere:

  1. De arts houdt zich eerder aan geschreven richtlijnen om een beslissing te nemen.
  2. Artsen laten niet graag patiënten beslissen waarvan ze denken dat deze dit niet kunnen, zoals mensen die minderbegaafd zijn or uit een lagere sociale klasse komen.
  3. Vaak denken artsen dat ze wel gezamenlijke tot een beslissing komen terwijl dit niet zo is.
  4. Dikwijls gaat men er verkeerdelijk van uit dat de patiënt liever wil dat de arts beslist.
  5. De meeste artsen geven aan dat er simpelweg geen tijd is om op deze manier te werken, terwijl onderzoek heeft aangetoond dat de consultatie niet perse langer duurt.

En wat met België?

Reeds genoemde landen proberen deze attitudes van artsen positief te beïnvloeden door onderzoek te financieren, trainingen te voorzien en keuzehulpen te ontwikkelen. Het onderzoek in andere landen, waaronder België is echter erg beperkt. Het is daarom dus volstrekt normaal dat u als lezer over dit onderwerp nog niets eerder gehoord hebt. Deze masterproef had daarom als hoofddoel om na te gaan hoe het nu gesteld is met de attitudes van artsen en laatstejaarsstudenten geneeskunde aangaande gezamenlijke besluitvorming. Hiernaast werden ook masterstudenten verpleegkunde bevraagd. De attitudes van deze personen werden gemeten aan de hand van een korte vertaalde versie van de Patient Practitioner Orientation vragenlijst. Aan de hand van negen stellingen krijgt men een score waarin men kan stellen dat de deelnemer ofwel meer paternalistisch optreedt (als een ‘vader’ voor de patiënt beslist) of de patiënt betrekt in de besluitvorming.

Een totaal van 266 personen vulden deze vragenlijst in, waaronder 93 artsen, 147 studenten geneeskunde en 26 studenten verpleegkunde. Over het algemeen zijn zowel de Vlaamse studenten als de artsen eerder paternalistisch ten opzichte van studies vanuit de Verenigde Staten. De Vlaamse artsen scoren wel beter dan diegene in Griekenland, Malaysia en Nigeria, terwijl de studenten geneeskunde beter scoren dan die van Griekenland, Singapore en Pakistan. Het zou hierbij goed kunnen dat we lager scoren omwille van onze cultuur, waarin er sprake is van een duidelijke hiërarchie. Hierbij is het normaal dat we luisteren naar een hogere persoon in rang, ook al zijn we het er niet altijd mee eens.

De reden waarom onze landgenoten zo laag scoren is vooral omdat men (1) ervan overtuigd is dat de arts dient te beslissen in de zorgverlening; (2) denkt dat de patiënt niet zelf informatie mag opzoeken omdat dit juist voor meer verwarring zorgt; en (3) vindt dat patiënten de kennis van de arts moeten volgen en niet zelf informatie mogen opzoeken.

Naast deze hoofdzaken werden er nog enkele statistische testen uitgevoerd om na te gaan of het geslacht, leeftijd, keuze van het specialisme en beroep invloed had op de uiteindelijke score. Er werd geen verschil gevonden op vlak van leeftijd en beroep, maar wel op vlak van geslacht en keuze van het specialisme. Allereerst gaan vrouwelijke artsen en studenten geneeskunde eerder de patiënt betrekken in de besluitvorming. Ten tweede scoren artsen gespecialiseerd in pediatrie, psychiatrie, huisartsengeneeskunde en gynaecologie hoger dan chirurgen. Chirurgen staan met andere woorden een pak minder open voor gezamenlijke besluitvorming.

Opvallend was dat de studenten geneeskunde niet onderling verschillen afgaande op hun keuze van specialisme. Hierbij lijkt het erop dat het niet de persoonlijkheid en keuze van specialiteit is wat er voor zorgt dat men uiteindelijk minder de patiënt betrekt. Langdurige blootstelling aan de specifieke karakteristieken van de patiënten en de werkomgeving zou hier wel voor zorgen, zoals bijvoorbeeld op de spoedafdeling en het operatiekwartier.

Wat dient er te gebeuren?

Allereerst is het belangrijk dat zowel academische instellingen, werkgevers, patiëntenorganisaties als de overheid zich engageren om de implementatie van gezamenlijke besluitvorming te promoten. Op nationaal vlak heeft de overheid een belangrijk rol om mensen te beïnvloeden en hindernissen aan te pakken. Dit onderwerp staat echter nergens op de politieke agenda. Ten tweede dient men in te zetten om bestaande zorgverleners te trainen en het academisch curriculum van huidige studenten te versterken. In de masterproef komen tal van adviezen ter sprake hoe men dit het beste kan aanpakken. Ten derde dient er nog veel onderzoek te gebeuren naar hoe men gezamenlijke besluitvorming dient te implementeren, niet enkel in België maar in het algemeen.

Slot

Deze masterproef is de eerste die de attitudes van Vlaamse zorgverlener met betrekking tot gezamenlijke besluitvorming bestudeerd. Gezamenlijke besluitvorming is een van de belangrijkste elementen van een gezondheidszorg waarin met en voor de patiënt gewerkt wordt. België lijkt echter sterk achter te lopen op landen zoals de Verenigde Staten, Canada en zelfs Nederland, Frankrijk en Duitsland. De zorgverleners vinden immers nog steeds dat de arts dient te beslissen, dat de patiënt zelf geen informatie mag opzoeken en zich dient te houden aan de kennis van de arts. Om deze redenen dienen vooral beleidsmakers en academische instellingen initiatieven te nemen om van gezamenlijke besluitvorming een feit te maken. Dit zal ongetwijfeld een langdurig proces worden, maar een die nodig zal zijn om te kunnen streven naar en patiëntgerichte gezondheidszorg.
