De werking van ons lichaam wordt gezien als fenomenaal, maar er loopt soms ook wel eens iets mis. Dankzij technologische ontwikkelingen en het gebruik van screening en diagnostiek tijdens de zwangerschap, kunnen foetale afwijkingen al vroeg in de zwangerschap gedetecteerd worden. Waar vroeger enkel risicopatiënten een screening aangeboden kregen, is screening vandaag routine geworden bij elke zwangere. Ouders zien het resultaat van een test als een extra bevestiging van het ideaalbeeld dat ze hebben rond het ongeboren kind, maar staan er nog te weinig bij stil dat screening optioneel is en dat een eventueel negatief resultaat hun ervaring van de zwangerschap zal beïnvloeden.
“Voor dokters is het simpel, de diagnose is slecht, de zwangerschap wordt best zo snel mogelijk afgebroken. Maar voor een ouder is dat anders. Het is je kind waarvan je afscheid neemt. We hadden ook veel vragen. Hoe verloop de bevalling precies? Wat gebeurt er met ons kindje als ze wordt geboren? Het is zo belangrijk om eerst die antwoorden te krijgen.” Vertelt Tine Van den Bogaert die op 14 weken beviel van hun sterrenkindje Roos, trisomie 18.
Evolutie van screening en diagnostiek tijdens de zwangerschap
De NIPT (Niet Invasieve Prenatale Test) werd ingevoerd in april 2014 en wordt sinds juli 2017 volledig terugbetaald. Door deze evolutie wordt deze test routinematig uitgevoerd vanaf de 10de zwangerschapsweek. Via een bloedafname wordt er nagegaan of er een verhoogd risico aanwezig is op trisomie 21, trisomie 18 en trisomie 13, maar ook alle andere chromosomen worden gecontroleerd. Daarnaast wordt het DNA van de moeder geanalyseerd op mogelijke afwijkingen. Wanneer dit het geval blijkt te zijn, dan wordt ook dit resultaat meegedeeld.
Wordt er een chromosomale afwijking gevonden, dan wordt een invasieve of diagnostische test aangeraden. Ouders krijgen de keuze tussen een vlokkentest of een vruchtwaterpunctie. Wanneer deze test een afwijkend resultaat aangeeft, kan er in bepaalde gevallen geopteerd worden om de zwangerschap te onderbreken.
Door de evolutie van de screeningstesten, kunnen afwijkingen steeds vroeger en beter worden opgespoord, waardoor het aantal zwangerschapsafbrekingen ook toeneemt.
Zwangerschapsafbreking na 12 weken
Een zwangerschapsafbreking na 12 weken kan uitgevoerd worden wanneer de foetus lijdt aan een ‘uiterst zware kwaal’ die als ongeneeslijk wordt erkend op het ogenblik van de diagnose of wanneer de zwangerschap een gevaar inhoudt voor de gezondheid van de moeder.
Het begrip ‘uiterst zware kwaal’ wordt niet omschreven in de wetgeving waardoor de invulling ervan volledig aan de medische wetenschap overgelaten wordt. Een duidelijke grens bepalen tussen de afwijkingen die ernstig genoeg zijn en anderen die dit niet zijn wordt dus moeilijk.
Iedere casus wordt beoordeeld door het ethisch comité, hun advies is niet bindend maar heeft een grote gezagswaarde, waardoor artsen er niet snel zullen tegen ingaan.
Internet als informatiebron
Zowel op het werkveld als in hun persoonlijke omgeving komen zorgverleners in contact met koppels die geconfronteerd worden met een afwijkend resultaat bij prenatale screening en diagnostiek.
Het ouderpaar wordt door de zorgverlener overladen met informatie en kan deze vaak niet voldoende verwerken op het moment zelf. Onderzoek toont aan dat een groot deel van de koppels op het internet op zoek gaan naar informatie over een zwangerschapsafbreking. Op deze manier kunnen ze antwoorden vinden op hun vragen en deze informatie ook op hun eigen tempo verwerken.
Ze gaan op zoek naar informatie over de vermoedelijke aandoening bij de foetus, het verloop van een interruptie en proberen pagina’s te vinden waarop ze in contact kunnen komen met ervaringsdeskundigen.
Ook de anonimiteit van het internet zorgt ervoor dat de ouders hun schaamte overwinnen en dit biedt hen de kans om op deze manier pijnlijke gevoelens te onthullen. Een evidence-based website waar alle informatie rond prenatale screening en diagnostiek gebundeld staat voor ouders, was tot voor kort nog niet beschikbaar.
Het tot stand komen van de website ‘blijverwachtenwordtverdriet’
Gezien de noodzaak van een dergelijke website, besloten Ine Berghman en Cindy Casier hier aan te werken in het kader van hun bachelorproef. Na het uitvoeren van de literatuurstudie, werden de belangrijkste hoofdstukken rond dit thema geselecteerd. Op deze manier werd de informatie voor de website gebundeld en werd ten slotte de website gecreëerd. De website heeft als doel om te voldoen aan de grote informatiebehoefte die ouders hebben wanneer ze geconfronteerd worden met afwijkende resultaten na prenatale testen.
De website wordt midden oktober 2018 gelanceerd.
