Huid-op-huidcontact: Gelieve niet te storen!

Huid-op-huidcontact, wat is dat juist?

Huid-op-huidcontact is het eerste contact na de geboorte tussen de ouders en hun pasgeboren kind. Het is een uniek, ‘once-in-a-lifetime’ moment dat gegarandeerd aandacht verdient. Uit de literatuur blijkt dat zowel moeder als kind baat hebben hierbij. Het is belangrijk om stil te staan bij de vele voordelen en belangen dat dit te bieden heeft. Het werkt zowel fysiologische processen zoals stabilisatie van de parameters in de hand, alsook de hechting tussen ouders en pasgeborene. Mijn volledige bachelorproef werd gewijd aan de voordelen hiervan, doch ondervond ik uit literatuuronderzoek nog vaak dat een scheiding tussen moeder en kind na de geboorte een reëel feit is. Met dit eindwerk hoop ik interesse en aandacht op te wekken voor dit unieke moment.

De basis van deze bachelorproef steunt op twee belangrijke onderzoeksvragen:

‘Wat is het belang van huid-op-huidcontact tussen moeder en neonaat onmiddellijk na de partus?’

‘Hoe kan de partner participeren in de contactmomenten met de neonaat onmiddellijk na de partus?’

Leidraad

De rode draad doorheen dit werk is het aantonen van de vele belangen van huid-op-huidcontact kort na de geboorte. Er werd besproken wat huid-op-huidcontact precies inhoudt, hoe alles in zijn werk gaat vlak na de geboorte en wat nu juist die voordelen zijn die er onlosmakelijk aan verbonden zijn. Verder wordt aangehaald hoe dit contact gegarandeerd kan worden bij prematuren of bij een keizersnede.

Voornaamste resultaten

VoordelenDat huidcontact na de geboorte tussen moeder en kind heel wat voordelen heeft, werd ondertussen wel al gesteld. Hier volgen de meest voornamelijke voordelen ten gevolge van huid-op-huidcontact.

Huidcontact versus de nageboorte

Wanneer een vrouw een gewone, vaginale bevalling achter de rug heeft, staat haar nog een volgende fase in de bevalling te wachten. De placentaire fase, ofwel de nageboorte van de placenta of moederkoek, vangt enkele minuten na de bevalling van het kind aan. Uit onderzoeken blijkt dat wanneer de moeder haar pasgeboren kindje bij zich neemt, dit positieve invloeden heeft op de duur van de nageboorte. Mede dankzij verhoogde vrijstelling van hormonen, een meer optimale temperatuur en verminderde stress. Een verkorte nageboorte is uiteraard veel aangenamer en de kersverse moeder is hierdoor ook sneller op haar gemak.

Huidcontact versus de parameters

Huid-op-huidcontact is bewezen een degelijke behandeling te zijn voor allerlei kleine en grotere aandoeningen. Zo werd bevonden dat de hartslag en de ademhaling rustiger en stabieler bleven wanneer het kindje in huidcontact mocht liggen met de moeder. Alsook het zuurstofpercentage, de temperatuur, de bloedsuikerwaarde en het gewicht kwamen sneller tot een optimale waarde en bleven ook veel stabieler in vergelijking met kindjes die niet bij de moeder mochten liggen. Deze kindjes verbleven vaak in een bedje of in een couveuse indien ze iets te vroeg geboren werden. Ook de pijn bij kinderen die bij de moeder verbleven, was beduidend minder. Dit alles kan tot een gezamenlijke conclusie leiden: kinderen die bij de moeder mogen verblijven, ervaren gemiddeld minder stress dan kinderen die dit huidcontact missen.

Huidcontact versus slaap en wenen

Deze verminderde stress zal ook de slaap van het kind ten goede komen. Het kind zal minder wenen en dus een betere slaap ervaren, waardoor ook de moeder een minder stresserend gevoel zal ervaren.

Huidcontact versus borstvoeding

Kinderen met huidcontact bleken gemiddeld sneller tot een initiatie van borstvoeding te komen. Dit wil zeggen dat deze kindjes sneller naar de borst/tepel van de moeder reiken en sneller de zuigreflex vertonen, waardoor de borstvoeding eerder tot stand zal kunnen komen. Wanneer een kindje eerder zal proberen, zal vaak ook eerder succes waargenomen worden. Wanneer een kindje succes ervaart (en dan niet alleen het kindje, maar ook de moeder), zal het gemiddeld langer blijven proberen, wat voor een langere duur van voeding zorgt. Problemen die moeder en kind kunnen ondervinden zijn bijvoorbeeld het moeilijk aanhappen van het kind aan de tepel. Wanneer de tepels niet stijf genoeg zijn voor het kind om degelijk aan te kunnen zuigen, kan eventueel een tepelhoedje de oplossing zijn. Als laatste is exclusiviteit een beduidende meerwaarde. Dit wil zeggen dat het kind geen aanvullende voeding naast borstvoeding ontvangt, zoals flesvoeding of water. Huidcontact wordt geassocieerd met een langere periode van exclusiviteit.

Huidcontact versus hechting

Misschien de meest voor de hand liggende uitkomst: hechting tussen ouders en kind. Wanneer moeder of vader hun kindje bij zich pakken, is het niet onwaarschijnlijk dat de band tussen beide zal aansterken.

Huidcontact versus hormonen en reukzin

Vrouwen en hun hormonen: zo klaar als een klontje! Het is een feit dat de hormonen heel wat schommelingen ondergaan gedurende de zwangerschap en bevalling. Huidcontact zorgt alleen maar voor een positieve invloed hierbij. Catecholamines, oxytocine, prolactine en cortisol: de vier voornaamste, respectievelijk verantwoordelijk voor reactiviteit, gelukkig gevoel, melkproductie en stress. Ook de reukzin van het kindje kent positieve invloeden van huidcontact.

Huidcontact versus post-partumdepressie

Zoals eerder beschreven: meer contact zorgt voor verminderde stress. Het feit dat de hechting tussen moeder en kind positief beïnvloed wordt, zorgt er ook voor dat een post-partumdepressie minder zal opkomen.

Prematuur en keizersnede

Anders dan gewoon: een te vroeg geboren kind en een bevalling via een keizersnede. Uit onderzoek blijkt dat een prematuur perfect stabiel kan blijven dankzij huidcontact met de moeder. Alsook bij een keizersnede dienen er enkele aanpassingen in achting genomen te worden. Huid-op-huidcontact biedt de ideale oplossing.

Conclusie

Huid-op-huidcontact dient nagestreefd te worden.

Het eerste uur na de geboorte wordt erkend als ‘het heilige uur’. In het onderzoek van Philips (2013) wordt huid-op-huidcontact vergeleken met een huwelijksceremonie. Hierbij is het ongewoon om het bruidspaar te storen, zo stellen zij dit ook met het eerste contact tussen ouders en kind na de geboorte.

“No one would think of interrupting a wedding ceremony to give the bride and groom details about the flight arrangements for their honeymoon. Everyone recognizes that this information can wait until after the ceremony is completed. Birth is another sacred event. It is a time when a new member of the family arrives, is greeted for the first time and welcomed by his or her parents.” (Philips, 2013)
