Effortful control als modererende factor tussen reactieve temperamentsfactoren en psychopathologie bij psychogeriatrische patiënten

Temperament, je kracht of je achillespees?

Ooit, ooit word je oud. Wellicht hoop je dat je gespaard blijft van psychopathologie, maar wat als? De realiteit is dat 75-plussers ten opzichte van de algemene bevolking meer te kampen krijgen met psychische moeilijkheden. Garanties zijn er dus niet, maar wat als jouw temperament een belangrijke rol speelt op jouw oude dag?

Één temperament, verschillende componenten

Vaak wordt er gesproken over “hét temperament”. Echter kan dit volgens Rothbart onderverdeeld worden in twee componenten: het reactief en het regulatief temperament.

“Mijn peuter kan niet stilzitten”, “onze baby is zo (on)rustig” of “mijn kleuter is snel bang”. Het zijn uitspraken die je vast al gehoord hebt, het zijn allemaal uitingen van het reactief temperament. Dit is het primitieve gedeelte en maakt deel uit van een emotionele reactie. Volgens Gray kan het onderverdeeld worden in het Behavioral Inhibition System (BIS) en het Behavioral Activation System (BAS). Het doel van je BIS is om negatieve gevolgen te vermijden, dit door je gedrag te verhinderen. Het wordt in gang gezet wanneer je je in onaangename of angstaanjagende situaties bevindt, dan ga je angst voelen. Je BAS zal geactiveerd worden wanneer een situatie een positieve betekenis heeft. Je zal dan doelgericht reageren door toenadering te zoeken. Je voelt positieve emoties en zal impulsief zijn.

Je raadt het al, op deze emoties en gedragingen moet je uiteindelijk grip krijgen. Dat is de rol van je regulatief temperament, de zogenaamde Effortful Control (EC). Langzaam maar zeker zal deze de controle nemen over je reactief temperament. Deze zelfregulatie zorgt ervoor dat je een dominante reactie kan stopzetten, om een minder dominante reactie te stellen. Hiervoor heb je drie mechanismen: (1) de controle over je aandacht, zijnde het vrijwillig focussen en verzetten ervan, (2) de controle om gedrag niet te stellen door middel van een inspanning en (3) de controle die ervoor zorgt dat je ook gedrag stelt bij gebrek aan motivatie.

Paarden en ruiters

Je stelt je vast de vraag wat temperament met psychopathologie te maken heeft. Het BIS hangt samen met angst en het BAS met impulsiviteit. Wanneer het BIS of het BAS zoals een paard op hol slaat ontstaan psychische klachten of psychopathologie. Voor het BIS betreft het internaliserende klachten of psychopathologie zoals onder andere een sombere stemming, een depressieve of een angststoornis. Het BAS gaat gepaard met externaliserende klachten of psychopathologie bijvoorbeeld, gedrags- of impulsiviteitsproblemen, ADHD of middelenmisbruik. Het duale procesmodel stelt dat een onevenwicht tussen het reactief en het regulatief temperament psychopathologie kan uitlokken. Een hoog regulatief temperament zou dan een beschermende factor zijn in geval van een te hoge BIS of BAS. Bij wijze van spreken kan een krachtige ruiter (cfr. regulatief temperament) het op hol geslagen paard (cfr. reactief temperament) intomen. Daarentegen verhoogt de kans op psychopathologie bij een hoog reactief temperament met een laag regulatief temperament. Met andere woorden, wanneer de ruiter zwak is zal deze het paard niet onder controle krijgen. De ruiter kan, door onder andere cognitieve training, sterker gemaakt worden. Wat kan leiden tot een vermindering van symptomen, het paard zal minder hard op hol slaan. Moraal van het verhaal: de ruiter en het paard moeten evenwichtig op elkaar inspelen.

Ouder worden, we kennen allemaal de grijze haren en rimpels. Je hersenen veranderen ook, zo sterven er hersencellen af. Dit gebeurt ook in het deel van de hersenen dat zich vlak achter je voorhoofd bevindt, de zogenaamde frontale kwab. Net dat deel is cruciaal voor zelfregulatie. Hierdoor ben je als oudere mogelijk vatbaarder voor problematieken gelinkt aan een verminderde zelfregulatie en bijgevolg meer psychopathologie.

De duale procestheorie bevestigd?

Onderzoek naar psychopathologie bij ouderen is eerder schaars. Er wordt nog vaak gedacht dat de behandeling van psychopathologie op oudere leeftijd geen zin meer heeft of onmogelijk is. Dit ondanks de vergrijzing, de grote mate van psychisch leed en de gevolgen van psychopathologie in deze groep. Daarenboven is er ook bij oudere psychiatrische patiënten vaak sprake van herval en heropnames, wat ons doet vermoeden dat er iets ontbreekt in het zorgaanbod. Voorgaande argumenten maken onderzoek naar deze populatie uiterst belangrijk. De duale procestheorie werd nog niet onderzocht bij psychogeriatrische patiënten, waardoor dit de focus van de masterproef was. Aan de hand van vragenlijsten werden het regulatief temperament (EC), het BIS, het BAS en de mate van psychologische distress en psychopathologie in kaart gebracht. Na het uitvoeren van een statistische analyse bleek de dualeBevindingen masterproef procestheorie bevestigd. Een laag regulatief temperament met een hoog reactief temperament gaat steeds gepaard met een ernstige mate van psychopathologie. Een hoog regulatief temperament bleek bij een hoog reactief temperament bescherming te bieden tegen ernstige mate van psychopathologie. Meer specifiek leidde een hoge BIS samen met een hoge zelfregulatie tot minder internaliserende psychopathologie, dan wanneer de zelfregulatie laag is. Ook een hoge BAS samen met een hoge zelfregulatie leiden tot minder externaliserende psychopathologie. Het plaatje van het regulatief temperament, zijnde de sterke ruiter, als beschermende factor tegen ernstige psychopathologie werd in de thesis bevestigd. Ondanks dat dit fenomeen veelvuldig onderzocht werd bij kinderen en volwassenen is dit de eerste maal dat dit in deze populatie bevestigd werd. Daarenboven geldt deze bevinding omtrent het temperament over veel verschillende psychopathologieën heen. Wat een belangrijke troef is in een populatie die vaak kampt met meerdere psychopathologieën tegelijkertijd. Het leidt ertoe dat er een beter begrip kan komen met betrekking tot psychopathologieën, maar ook dat er vooruitgang gemaakt wordt op vlak van psychodiagnostiek en therapie.

De bevestiging van de duale procestheorie leidt impliciet tot de conclusie dat cognitieve trainingen mogelijk een waardevolle aanvulling op het bestaande behandelingsaanbod zijn. Hier moet de wetenschap zich in toekomstig onderzoek nog verder over buigen. Doch luidt de conclusie van huidig onderzoek dat een stevig regulatief temperament belangrijk is om een goede psychische gezondheid te waarborgen. De vraag van één miljoen: zou jouw temperament je kracht of je achillespees zijn in geval van psychologische moeilijkheden op je oude dag?
