Het scherm en ik. Een kwalitatief onderzoek naar de ervaring van therapeuten met online therapie.

In de zetel zakken met pyjamabroek aan en de patatjes op het vuur? Drie redenen waarom online therapie geven misschien toch niet zo ‘chill’ is als het lijkt.

We reizen even terug naar 2020. Volle coronacrisis. U zit achter uw computer voor de zoveelste zoommeeting van de dag. U wrijft in uw ogen en zucht wanneer u eindelijk het beeldscherm kan uitzetten. Gelukkig kunnen we vandaag elkaar weer face-to-face ontmoeten. Toch is een dag vol videogesprekken voor een groeiend aantal therapeuten de dagelijkse realiteit. Sinds de COVID-19 pandemie is het gebruik van videobellen voor therapie namelijk gestegen.

Recente bevindingen tonen aan dat deze vorm even effectief kan zijn als traditionele therapie. Studies laten echter de beleving van therapeuten grotendeels onderbelicht. De motivatie en het welzijn van deze groep is nochtans belangrijk voor de kwaliteit van psychotherapie.

Therapie via videobellen verschilt op een aantal aspecten duidelijk van traditionele therapie. Om het welzijn van therapeuten te bewaren, moeten we hiervoor aandacht hebben bij de organisatie van online therapie. Hierop focussen in opleidingen kan dit ondersteunen. Onderzoek naar de beleving is dan belangrijk om opleidingen te helpen vormgeven.

Zei er iemand Zoom Fatigue?

In interviews met negen psychologen gingen we dieper in op hun ervaringen met online therapie. Naast een aantal praktische voordelen, zoals tijdsbesparing en het comfort van de eigen thuisomgeving, kwam tijdens het onderzoek de vraag op of online werken wel even goede werkomstandigheden toelaat als traditionele therapie. De werkomstandigheden blijken voornamelijk een uitdaging wanneer ze therapie van thuis uit geven, maar beperkt zich hier niet toe.

Het online medium heeft karakteristieken die ideale werkomstandigheden bemoeilijken. Zo geven de therapeuten aan minder te bewegen, minder rond te kijken en een stijvere houding aan te nemen bij therapie via videobellen. Ze rapporten hierbij ook meer vermoeidheid en/of meer afleiding tijdens die online sessies.

Daarnaast blijkt de privé-werkgrens ook moeilijker te bewaken bij therapie van thuis uit. Sommige therapeuten laten zich sneller verleiden tot avondwerk, anderen geven aan dat je thuis minder controle hebt over wat je toont van jezelf. Zo vergeten ze een partnerfoto weg te halen of komt bol.com net op het verkeerde moment.

“Het is een beetje een vergiftigd geschenk en niet zo gezond. Je moet ergens wel grenzen stellen. Ik werk al twee avonden in de week hier, dus als je dan nog een avond ook thuis werkt, dan ben je al drie avonden van de week aan het werken. Hoeveel tijd heb je dan nog voor jezelf?”

Ik zie je toch liever in het echt.

Zoals reeds aangehaald blijkt dat online therapie even goed werkt als traditionele therapie. Ook de therapeuten uit deze studie geven aan dat ze verbeteringen zagen bij hun patiënten. Toch verkiezen de meesten hun patiënten in levenden lijven te zien. Een handdruk aan de deur, babbeltje terwijl men naar de therapiekamer wandelt, geur of klederdracht zijn slechts een deel van de menselijkheid die therapeuten missen online.

“Wij zijn sociale wezens en we zijn nooit anders gewoon geweest dan bij elkaar te komen. Dus ook al zijn die gesprekken online even goed, toch verkies ik nog altijd van mensen te zien. Zeker als ze persoonlijke dingen komen vertellen.”

Daarnaast gaat via het beeldscherm ook een deel non-verbale communicatie verloren, zoals handgebaren, houding en zelfs voetbewegingen. Waar stiltes in traditionele therapie een belangrijke moment kunnen zijn, ervaren therapeuten die anders online. Ze geven aan dat ze bij online stiltes meer kilte en afstand ervaren en deze sneller doorbreken.

Heb je mij goed gehoord?

Ten slotte brengt online therapie meer zorgen en onzekerheid met zich mee. Zo zijn sommige therapeuten onzeker wanneer ze met online therapie beginnen. Ze weten niet of het therapieproces op dezelfde manier verloopt, hoe de patiënten het beleven en of het dezelfde verandering teweeg kan brengen.

“Ik merkte dat ik in het begin veel meer de neiging had om de gesprekken voor te bereiden om wat richting te geven en wat te kunnen sturen. Terwijl dat eigenlijk een beetje mijn onzekerheid was voor het proces. Ik vroeg me af of het op dezelfde manier ging verlopen of niet.”

Daarnaast brengen storingen in de verbinding twijfel met zich mee. Het is soms onduidelijk of een patiënt iets niet goed begrepen heeft omdat er ruis op de verbinding zit of omdat die door zijn moeilijkheden de vraag niet goed kan begrijpen. Verder weet een therapeut soms niet of er een stilte valt of dat de verbinding blijft hangen.

Als laatste zijn de fysieke afstand en de beperkte non-verbale communicatie een bron van onzekerheid. Therapeuten weten niet of de patiënt wel in een veilige situatie zit, of die wel alleen is in de kamer en of die niet iets anders doet tijdens de therapie. Daarbij komt dat ze het moeilijker vinden om aan te voelen hoe hun patiënt zich voelt en in welke staat ze deze achterlaten wanneer ze de sessie afsluiten. Tot slot vinden therapeuten het moeilijker het effect van hun interventies in te schatten online.

Waarom zijn deze bevindingen belangrijk?

Therapeuten die online therapie geven, kunnen aandacht besteden aan deze bevindingen bij de organisatie van hun werk. Zo lijkt het beter om online therapie toch vanuit een therapieruimte te geven, online sessies af te wisselen met face-to-face sessies en opleiding te volgen rond online therapie om onzekerheid weg te nemen.

Er is echter nog verder onderzoek nodig voordat we deze aanbevelingen met zekerheid kunnen doen. Vooreerst dient belevingsonderzoek verder uitgebreid te worden. Daarnaast kan onderzoek oplossingen voor de beschreven problemen evalueren. Zo kan men verschillende camerahoeken uitproberen om meer non-verbale communicatie toe te laten en nagaan of de privé-werkgrens beter behouden blijft als therapeuten vanuit een therapieruimte werken.

Tot slot kunnen aanbevelingen uit dergelijk onderzoek de klinische praktijk en de vormgeving van opleidingen informeren. Dit om zowel de kwaliteit van online therapie als het welzijn van de therapeuten te verbeteren.

Want zeg nu zelf, iemand moet toch eens aan de therapeut vragen: “En? Hoe voelt u zich daarbij?”.
