Over taboes gesproken: psychiatrie en seksualiteit
Seksualiteit is een complex gegeven dat steeds meer belangstelling krijgt in de media, politiek en de privé sfeer. Nochtans is het stigma hier omtrent nog niet verdwenen. Onder andere wanneer seksualiteit gekoppeld wordt aan psychiatrische hulpverlening heersen er nog verschillende vooroordelen, een gebrek aan kennis en vooral: een buitengewoon taboe.
Tot op heden wordt er in zowel de reguliere media, als in wetenschappelijk onderzoek nog te weinig (positieve) aandacht geschonken aan de seksualiteitsbeleving van personen met een psychische kwetsbaarheid. Seksuologe Chayenne Krupa gaf personen met een psychotische stoornis een stem in haar masterproef. De participanten deelden hun verhaal over het onbesproken onderwerp bij uitstek: seksualiteit. Hierbij mocht Chayenne steun ontvangen van haar promotor en tevens geen onbekende in de wereld van de psychiatrie, professor Erik Thys.
Doorheen de interviews werden verschillende ervaringen en gedachtes gedeeld. Er werd zeer openhartig en rauw verteld over een thema waar velen nooit eerder zo diepgaand over mochten spreken. Wat volgt is een summiere samenvatting van de resultaten.
De diagnose, een donkere wolk?
Doordat de eerste psychotische symptomen bij de deelnemers reeds optraden tussen 17 en 30 jaar, voelden zij een enorme belemmering in de ontwikkeling van hun seksueel functioneren. Dit gaat namelijk net om een leeftijd waarin sociale vaardigheden en seksualiteit volop ontwikkelen en waar vaak de eerste stappen worden genomen naar intieme relaties. Dit gebrek aan duurzame relaties en intimiteit zorgt ervoor dat velen een negatieve seksuele identiteit ontwikkelen en te maken krijgen met gevoelens van eenzaamheid.
“Ik heb twee psychoses gehad en dat was twee keer tijdens die psychose gebeurd dat ik, euhm, mij afzonderde van de wereld. En wat dat mij vooral bijblijft is dat ik toen het eenzaamste was dat ik ooit bent geweest. En dat ik toen ook wel echt opzoek ging naar intimiteit, maar dat dat mij niet echt lukte.” - Deelnemer 1
Vormgeven van je seksualiteit
Zoals hierboven werd beschreven, werden de deelnemers van het onderzoek al op jonge leeftijd gediagnosticeerd. Door de diagnose, de symptomen en de moeilijkheden die het met zich meebrengt, voelden zij zich meer belemmerd in hun seksuele ontwikkeling en bouwden zij minder seksuele ervaringen op. Toch blijken zij naar seksuele intimiteit te verlangen en erover te fantaseren. Hun seksuele behoeftes verschillen daarin weinig van de algemene bevolking. Overwegend bleken deelnemers een negatieve seksualiteitsbeleving te hebben en beschreven ze seksualiteit vooral als een groot gemis. Hierin spelen (zelf)stigma en het daardoor ontstane negatieve zelfbeeld een grote rol.
“Als achttien/twintig jarige zag ik mezelf wel aan het klassieke beeld voldoen, met ook kinderen. En dan, ja ik ben zelf veel bezig met zelfstigma, dat ik dacht van; ik wil dat kind dat niet aandoen, zo een vader als ik.” – Deelnemer 2
Spreken over seks(ualiteit)
Spreken over seks en seksualiteit blijkt niet steeds evident te zijn. Deelnemers gaven wisselend aan of en wanneer ze erover spreken met hulpverleners en personen uit hun omgeving. De meesten gaven aan er niet spontaan over te praten. Nochtans blijkt ondersteuning door zorgverleners wel positieve effecten te hebben. Zo kan de juiste begeleiding ervoor zorgen dat personen meer zelfvertrouwen en een positiever zelfbeeld ontwikkelen.
Niet te vergeten… de rol van het zorgsysteem
De seksuele noden van personen met een psychotische aandoening worden zoals hierboven staat beschreven vaak onderschat en hulpverleners hechten nog te weinig belang aan dit onderdeel van personen hun functioneren. Deelnemers aan het onderzoek gaven aan dat ze het moeilijk vinden om dit onderwerp zelf ter sprake te brengen, wat stimulatie en ondersteuning van zorgverleners dus extra belangrijk maakt. Daarnaast benadrukten de deelnemers de meerwaarde van individuele- en groepsgerichte hulpverlening rond dit thema.
Conclusie
Er is duidelijk sprake van een eerder negatieve beleving van de seksualiteit bij personen met een psychotische stoornis. Een algemeen gebrek aan kennis, vaardigheden en ervaring belemmeren een positieve ontwikkeling van de seksualiteit. De seksuele identiteit wordt gekleurd door zelfstigmatisering, angst en onzekerheid.
Hoewel het onderzoek slechts vijf deelnemers kende, leveren de resultaten belangrijke inzichten. Deelnemers kregen de kans om hun verhaal in eigen handen te nemen en open te vertellen over ervaringen, gedachten en gevoelens die zij niet eerder op die manier deelden.
Daarnaast biedt de masterproefstudie veel mogelijkheden voor toekomstig onderzoek. Aangehaalde thema’s en hun onderlinge relaties kunnen verder worden uitgewerkt om zo de gevonden resultaten te verdiepen, aan te vullen en te verfijnen.
Ten slotte mag het onderzoek hopelijk een inspiratie betekenen voor hulpverleners om dit thema meer bespreekbaar te maken, om zo mee het taboe te doorbreken en aan te tonen dat seksualiteit een aspect is dat in ieders leven aanwezig mag en kan zijn, in welke vorm dan ook.
