Ouderschap voorbij de norm: een ouder ben je niet door je genen

Een cisgender vrouw en een genderdiverse persoon met een droom om samen het ouderschap op te nemen. Op de fertiliteitsdienst houden ze een informatiemap vast waarop te lezen staat: “Vrouwenkliniek”. “Je schaamt je dan bijna” getuigt een trans mannelijke deelnemer in een onderzoek naar de ervaringen van genderdiverse koppels binnen een fertiliteitstraject. Het is de kern van een pijnlijke, realistische paradox: trans en non-binaire personen moeten hun weg naar ouderschap navigeren in een medische wereld die nog strak is ingericht volgens binaire hokjes van ‘man’ en ‘vrouw’.

Ongeveer de helft van de genderdiverse personen koestert een kinderwens, maar hun weg naar ouderschap is bezaaid met medische en maatschappelijke hindernissen. Uit het onderzoek van Maximo Meerschaut, student aan de interuniversitaire masteropleiding Gender en Diversiteit, blijkt dat Belgische fertiliteitscentra nog te vaak binair zijn waarin trans en non-binaire personen zich letterlijk en figuurlijk niet thuis voelen. Het is een emotionele rollercoaster waarbij je voortdurend moet navigeren tussen je identiteit en je kinderwens. Hierbij is het open communiceren naar elkaar toe als koppel, maar ook naar de zorgverleners toe cruciaal.

Voor de meeste mensen is de stap zetten naar de fertiliteitsdienst zwaar, maar voor genderdiverse personen komt hier nog een extra complexiteit bij. Meerschaut interviewde achttien ervaringsdeskundigen en analyseerde de beleidsprotocollen van zes Belgische fertiliteitscentra. De zorgsector is, hoewel medisch hoogstaand, nog sterk verankerd in cisgenderisme en pronatalisme, waarbij men ervan uitgaat dat de wereld enkel bestaat uit ‘biologische’ mannen en vrouwen die allemaal een kinderwens koesteren.

De biologische klok

Trans en non-binaire personen staan vaak voor de aartsmoeilijke keuze, de zogenaamde biologische tijdklem, waarbij deze groep moet kunnen tussen zichzelf zijn door middel van hormonale therapieën of medische ingrepen, of het uitstellen van hun transitie om hun vruchtbaarheid te redden. Beslissingen over hun nageslacht moeten vaak genomen worden op het moment dat hun focus volledig op hun genderzoektocht ligt, legt Meerschaut uit. Hun reproductieve autonomie wordt vaak onder druk gezet door onder andere de niet terugbetaalde procedure van het invriezen van eicellen.

“Mevrouw” in de wachtzaal

Tijdens de transitie, maar ook zeker tijdens een fertiliteitstraject botsen genderdiverse personen regelmatig op structurele, vaak expliciet vrouwelijk georiënteerde muren. Je krijgt een map mee van het ziekenhuis waar in het groot ‘Vrouwenkliniek’ op staat. Dan voel je je als man bijna beschaamd, getuigt een van de participanten.

Ook in de communicatie loopt het vaker mis en worden trans en non-binaire personen in de wachtzaal of tijdens de medische onderzoeken nog stelselmatig aangesproken als ‘mevrouw’. Voor velen leidt dit tot een versterking van hun reeds aanwezige genderdysforie. Medische procedures, zoals vaginale echo’s, worden vaak ervaren als een pijnlijke confrontatie met hun ongewenst lichaam.

Liefde maakt je een ouder, niet je genen

Opvallend in het onderzoek is hoe deze genderdiverse koppels naar verwant- en ouderschap kijken. Waar de maatschappij nog vaak hamert op de ‘bloedband’, bleek dit voor de meeste participanten ondergeschikt aan emotionele hechting en opvoeding. “Opvoeding en omgeving staan centraal, niet de biologie”, klinkt het. Hierbij wordt een onderscheidt gemaakt tussen een biologische kinderwens en een kinderwens, aldus een van de participanten.

Het ouderschap wordt ook meer egalitair ingevuld, waarbij deze gezinnen breken met de traditionele, binaire rolpatronen van ‘moeder’ en ‘vader’. Hiervoor creëren ze hun eigen taal en normen, voorbij de bestaande hokjes. Beide ouders zijn evenveel betrokken, ongeacht hun genderidentiteit. Ze beleven het ouderschap als positief, betekenisvol en verbindend.

Genderdiverse koppels worden in de bredere samenleving vaak nog geconfronteerd met ongepaste vragen of beleids-en juridische systemen die uitsluiting veroorzaken.

Tijd voor verandering: de factsheet en het kinderboek

Om de kloof tussen de medische wereld en de realiteit van deze genderdiverse koppels te verkleinen, ontwikkelde Meerschaut een concrete factsheet gericht naar fertiliteitscentra.

Hierin werd het hanteren van neutrale, inclusieve taal op documenten, het aanbieden van dysforie beperkende onderzoeken en dubbele tijdsloten aanbevolen. Net zoals het versterken van de basiskennis, het bewaken van professionele grenzen en het actief betrekken van de partner. Maar daar stopt het niet, voor kinderen die opgroeien in deze bijzondere gezinnen, ontwierp Meerschaut een inclusief kinderboekje “Ons bijzondere gezin”.

De kernboodschap van het onderzoek

Het verlangen naar ouderschap is een universeel gegeven, maar de reis ernaartoe blijkt niet voor iedereen gelijk te zijn. Het is cruciaal en tijd dat onze zorgsystemen niet langer vragen aan genderdiverse personen om hun identiteit ‘even te parkeren’ in ruil voor een kind, maar dat de zorg de mens achter de patiënt volledig erkent.
