Biomechanical evaluation of a personalized external aortic root support applied in the ross procedure

GEPersonaliseerde Aorta STeun, GEPAST?

Biomechanische analyse van het gebruik van een gepersonaliseerde aorta steun in de Ross procedure

‘E=mc²’ een gekende uitspraak van een mogelijk nog bekendere persoon. De wijze waarop Albert Einstein is overleden is een minder gekend aspect van zijn leven. Zeven jaar voor zijn dood werd hij het ziekenhuis binnen gebracht met een enorm aneurysma van de aorta (een uitstulping of dilatatie van de lichaamsslagader die indien niet behandeld, kan scheuren met een dodelijke afloop tot gevolg). De chirurg van dienst bedacht niets beter dan cellofaan papier rond dit aneurysma te wikkelen om zo de wand van de meest cruciale ader van het menselijke lichaam te verstevigen. Echter, zeven jaar na deze ingreep bezweek de versteviging en dit leidde tot de dood van één van de meest bekende geniën.

Zestig jaar na de dood van Einstein is de techniek om aneurysma’s van de aorta behandeld veranderd. Deze veranderingen zijn niet zo drastisch als men zou verwachten. Deze scriptie spitst zich toe op het onderzoek naar het gebruik van een recent ontwikkelde technologie, namelijk de GEPersonaliseerde Aorta Steun of GEPAST, ter behandeling van aneurysma’s. De GEPAST is een textiel dat de vorm van de gedilateerde aorta van de patiënt aanneemt, een soort maatpak voor de aorta, en rond het aneurysma wordt bevestigd. Deze techniek wordt reeds succesvol toegepast in patiënten met het Marfan syndroom. Het Marfan syndroom is een genetische ziekte waardoor het bindweefsel, zoals bijvoorbeeld aderweefsel, in het hele lichaam verzwakt. Deze patiënten lopen het risico op een gedilateerde of uitgezette aortawortel, de aanhechtingsplaats van de aorta met het hart. Wanneer deze dilatatie te groot wordt, is een operatie onvermijdbaar. Deze zeer invasieve operatie, waarbij het hart tijdelijk wordt stilgezet, bestaat uit het verwijderen van dit deel van de aorta en de aortakleppen en het plaatsen van mechanische kunstkleppen en een kunstmatig stuk aorta. Hierdoor is de  vaak jonge patiënt veroordeeld tot het levenslang nemen van bloedverdunners met bijhorende risico’s en beperkingen.

Ingenieurs in de geneeskunde

De GEPAST, het maatpakje voor de aorta, is ontwikkeld door de Britse  ingenieur Tal Golesworthy, zelf een Marfan patiënt. De operatie waarin het maatpakje rond de aorta wordt genaaid, is minder invasief en bijgevolg minder gevaarlijk dan het alternatief, aangezien het stilleggen van het hart niet meer nodig is. Bovendien is het nemen van bloedverdunners niet meer vereist waardoor de patiënt “medicatieloos” door het leven kan en een actievere levensstijl kan onderhouden. Reeds verschillende patiënten hebben deze procedure succesvol ondergaan, en Tal Golesworthy is zelf de eerste patiënt die de GEPAST geïmplanteerd kreeg in 2004. Elf jaar na de eerste implantatie van een GEPAST zijn de klinische voordelen en successen onmiskenbaar.

Op verkenning

Omwille van dit succes gaan er stemmen op om deze GEPAST nog in andere aneurysma-gerelateerde toepassingen te gaan gebruiken, zoals bijvoorbeeld de Ross procedure. De Ross procedure is een operatie die voornamelijk wordt uitgevoerd op jonge mensen, waarbij de verslechterde aortakleppen samen met een stuk aorta vervangen worden door de longslagaderkleppen en een stukje longslagader van de patiënt zelf. Deze longslagaderkleppen worden op hun beurt vervangen door de longslagaderkleppen van een donor. Echter, een veel voorkomend probleem bij deze procedure is de dilatatie van het stukje longslagader in aortapositie, waardoor vaak een heroperatie vereist is. Hierdoor passen vele chirurgen deze procedure niet toe, ondanks de vele positieve aspecten die hieraan verbonden zijn. Zo moet de patiënt geen bloedverdunners nemen en er is aangetoond dat de lichaamseigen longslagaderklep in aortapositie superieur is ten opzichte van de alternatieven, zoals donorkleppen, biologische of mechanische kunstkleppen. Kan de GEPAST de longslagader die geïmplanteerd is op aortapositie verstevigen?  Deze vraag vormt een uitgelezen kans om verschillende ingenieurstechnieken toe te passen op de huidige medische wetenschap.

Nog meer ingenieurs in de geneeskunde

Deze scriptie is het resultaat van een nauwe samenwerking tussen ingenieurs van de KU Leuven en chirurgen van het UZ Leuven. Dit onderzoek vormt een belangrijke stap in het gebruik van de GEPAST in de Ross procedure en opent de deur voor verder onderzoek naar het toepassen van de GEPAST in andere aneurysma-gerelateerde toepassingen. Bovendien is dit het eerste onderzoek dat sinds de uitvinding van de GEPAST de mechanische eigenschappen van deze versteviging rond de aorta en longslagader onderzoekt.

De mechanische eigenschappen van deze versteviging zijn van cruciaal belang. Beschouw de aorta als een langwerpige ballon die bij elke hartslag wat uitzet en vervolgens terug inkrimpt. Deze uitzetting en opeenvolgende inkrimping zorgen voor een goede stroming van het bloed. Wanneer de versteviging rond de aorta te stijf en te sterk is, te vergelijken met een aorta die in een PVC buis is geperst, limiteert deze versteviging de essentiële beweging van de aorta en induceert hoge spanningen in de aortawand. Deze hoge spanningen kunnen ongewenste reacties in de aortawand teweeg brengen. Daarentegen, wanneer de versteviging te zwak is, kan deze de nodige ondersteuning voor de gedilateerde aorta niet voorzien.

Het bepalen van de mechanische eigenschappen van deze versteviging vormt het experimentele luik van dit onderzoek. Het rekenkundige luik van het onderzoek is het gebruiken van deze eigenschappen in een model dat simuleert wat de invloed van de versteviging op de aortawand is. Dit model staat toe de invloed van de versteviging op de aorta te onderzoeken.

Echter, dit onderzoek staat in kinderschoenen en er is nog veel werk voor de boeg zowel voor ingenieurs als chirurgen. De resultaten van dit verkennend onderzoek zijn veelbelovend en de uitbreiding van de GEPAST naar de behandeling voor alle aneurysma’s is een kwestie van tijd. Zo kunnen in de verdere toekomst met behulp van deze innovatieve techniek de geniën van morgen gered worden.
