Overaanbod aan praktijkdierenartsen in België: feit of mythe?

De verzadiging aan praktijkdierenartsen op de Belgische arbeidsmarkt is reeds decennialang een heet hangijzer. Deze zogenaamde ‘plethora’ wordt beschouwd als bron van toenemende geografische concurrentie, met tariefconflicten, lage lonen, vroegtijdige loopbaanbeëindiging, en een hoog voorkomen van depressie onder praktijkdierenartsen als gevolg.

Tot dusver ontbraken echter gegevens om het werkelijke aanbod aan praktijkdierenartsen en hun activiteit te documenteren. Dit onderzoek werpt voor de eerste keer een objectief licht op deze problematiek.

Diergeneeskunde in België: een toevluchtsoord voor studenten uit de buurlanden

In België is er voor de opleiding diergeneeskunde noch een toelatingsproef, noch een numerus clausus (inschrijvingslimiet) vastgelegd. Onze buurlanden hebben wel instroom beperkende maatregelen ingevoerd, wat resulteert in een influx van buitenlandse studenten: studenten die geen toelating krijgen tot de opleiding in hun thuisland, proberen hun droom om dierenarts te worden alsnog te realiseren in België (Fig. 1).

Afhankelijk van hun moedertaal, kiezen Nederlandse en - in beperkte mate - Duitse studenten voor Vlaamse universiteiten, terwijl Franse studenten Waalse instellingen verkiezen.

De afgelopen decennia heeft de Waalse Gemeenschap de toelatingsvoorwaarden tot de opleiding diergeneeskunde verschillende malen aangepast om de instroom van studenten te beheersen. De Vlaamse Gemeenschap daarentegen heeft steeds gekozen voor een open toegang tot de opleiding diergeneeskunde. Naarmate de studentenpopulatie toeneemt, wordt het echter steeds moeilijker voor de academische instellingen om de kwaliteit van het onderwijs te handhaven, laat staan te verbeteren.

Uit verder onderzoek blijkt dat België een aanzienlijk grotere uitstroom aan dierenartsen heeft in vergelijking met de buurlanden.

Diergeneeskunde in de knoop met democratisering hoger onderwijs

Dit alles leidt tot de centrale vraag: moeten er maatregelen genomen worden om de instroom tot de opleiding diergeneeskunde in België te beperken?

Dit is een teer punt, aangezien open toegang tot onderwijs een fundamenteel democratisch principe is in België. Toch werd door de invoering van een toelatingsproef voor de opleiding geneeskunde en tandheelkunde hierop een uitzondering gemaakt om een overaanbod aan geneeskundigen te voorkomen. Er wordt immers aangenomen dat meer artsen per inwoner tot een hoger gebruik van (vaak onnodige) medische diensten leidt.

Hoewel de opleiding diergeneeskunde met eenzelfde problematiek kampt, was er weinig politieke wil om gelijkaardige maatregelen te treffen. Aangezien dierenartsenpraktijken niet gelinkt zijn aan de sociale zekerheid, heeft een overaanbod geen impact op de publieke financiën. Verder is de financiering van hogere onderwijsinstellingen deels afhankelijk van het studentenaantal. Een lager aantal studenten ten gevolge van de invoering van een toelatingsproef betekent dus minder facultaire inkomsten. 

Plethora aan praktijkdierenartsen in België?

Hoewel de meerderheid van studenten diergeneeskunde afstudeert als praktijkdierenarts, zal niet elke afgestudeerde in België als praktijkdierenarts aan de slag gaan. Sommigen starten in een buitenlandse praktijk of zoeken andere carrièremogelijkheden zoals industrie, onderwijs, onderzoek, volksgezondheid, enz.

Hoewel de overvloed aan praktijkdierenartsen op de Belgische arbeidsmarkt al langer bekend was, bleef  vóór deze studie het werkelijke aanbod aan dierenartsen in België onbekend. Bijgevolg was het ook onduidelijk of België in vergelijking met de buurlanden een groter aanbod aan dierenartsen heeft in verhouding tot het aantal dieren, veebedrijven en de totale bevolking.

Een ernstig probleem dat een gedetailleerde analyse van de praktijkdierenartsen in België in de weg staat, is de versnipperde en ongestructureerde verzameling van demografische gegevens, die zonder onderling overleg uitgevoerd wordt door meerdere instanties. Bijgevolg zijn de verzamelde data vaak moeilijk toegankelijk. Voor deze studie moesten dan ook gegevens uit meerdere bronnen samen gepuzzeld worden om een betrouwbaar beeld te verkrijgen.

Enkel voor Frankrijk en Duitsland bleek het mogelijk om een volledig overzicht te verkrijgen van de activiteit van de praktijkdierenartsen. Voor België en Nederland waren er minder details te vinden (Fig. 2).

Uit de vergelijkende studie, die de verhouding van het aantal praktijkdierenartsen tot het aantal dieren, veebedrijven en mensen per land naging, blijkt dat België veruit het hoogste aanbod heeft. Voor de eerste maal zijn er nu dus harde gegevens beschikbaar die het bestaan van een overaanbod aan dierenartsen op de Belgische arbeidsmarkt in vergelijking met onze buurlanden bevestigen.

De tarievenstrijd

Hoewel het hoger onderwijsbeleid in België een belangrijke invloed heeft op het aantal afgestudeerden, en bijgevolg op de arbeidsmarkt, zijn niet alle problemen waarmee de dierenartsenpraktijk kampt exclusief hieraan te wijten. Omdat in België de dierenartsenpraktijk onder de vrije beroepen valt, werken beginnende Belgische dierenartsen voornamelijk als zelfstandige, met bijhorende risico’s. Dit staat in contrast tot onze Noorderburen, waar jonge dierenartsen vaak starten onder de bescherming van een collectieve arbeidsovereenkomst.

Door het statuut als vrij beroep is elke Belgische dierenarts vrij zijn/haar tarieven te bepalen, wat bijdraagt aan de huidige tarievenstrijd. Deze situatie verschilt scherp met die van artsen, waar alle tarieven in de publieke sector strikt gereguleerd zijn door hun directe link met het sociale zekerheidsstelsel. In het verleden heeft de Orde der dierenartsen getracht een tarieflijst met verplichte minimumtarieven in te voeren, om zo een gezonde concurrentie op basis van kwalitatieve dienstverlening te stimuleren. Hoewel een gelijkaardige regeling reeds geruime tijd in voege is in Duitsland ('Tierärztegebührenordnung’), werd in België de Orde hiervoor gerechtelijk veroordeeld wegens inbreuken op de Belgische en Europese antikartelwetgeving.

Aanpak voor de toekomst

Een effectief beleid kan enkel gevoerd worden op basis van betrouwbare gegevens. Zoals hier aangetoond, is de beschikbaarheid van deze gegevens één van de knelpunten in België. Daarom is de ontwikkeling van een centrale databank van Belgische praktijkdierenartsen essentieel.

De twee Ordes der dierenartsen in België vormen de kern van de oplossing. Zoals in Frankrijk, zou een uniforme, gecentraliseerde gegevensverzameling door deze beroepsorganisaties de opbouw van een gedetailleerde nationale databank van alle actieve leden mogelijk maken. Integratie van deze databank met die van de federale overheid verzekert een eenvoudige toegang tot betrouwbare demografische gegevens, die als basis voor de beleidsvorming kunnen dienen.
