Persistent genital arousal disorder bij vrouwen: een systematische literatuurstudie

Onder het motto: het lichaam van de vrouw verdient meer aandacht.

We leven nog al te vaak in een mannenwereld en dat is ook te merken binnen onderzoek. Er zijn ontelbare artikels verschenen over problemen waarmee mannen te maken krijgen en dat is voor seksuele disfuncties niet anders. Mede daardoor zijn er voor mannen ook vaker oplossingen beschikbaar. Problemen om een erectie te krijgen? Neem een pilletje en de kans is groot dat je geholpen bent. 

Wanneer een vrouw met allerlei problemen kampt op seksueel vlak is dit vaak iets complexer en zijn oplossingen minder makkelijk voor handen. 

Vanuit het idee dat het lichaam van de vrouw meer aandacht verdient, ging ik op zoek naar een onderwerp voor mijn scriptie. Zo kwam ik bij Persistent Genital Arousal Disorder (PGAD) terecht, een aandoening waarbij een vrouw genitaal opgewonden is, zonder zich daarbij mentaal opgewonden te voelen. Eenvoudig gezegd: je bent heel vaak vochtig en voelt tintelingen daar beneden, maar toch heb je geen zin in seks. Met andere woorden: je hoofd zegt ‘nee’. Fysiek zijn er dus tekenen van opwinding aanwezig, maar er is geen seksueel verlangen. Het probleem wordt meestal ook niet (of slechts tijdelijk) verholpen door het krijgen van een orgasme en het wordt door de vrouw als ongewenst en onprettig ervaren. Vaak heeft dit ook psychologische en fysieke gevolgen voor de vrouw. De moeite waard dus om te onderzoeken.

Ik vroeg me bij de start van mijn onderzoek vooral af wat de oorzaak van deze aandoening zou kunnen zijn. Ik ging er vanuit dat bestaande artikels over het onderwerp mij snel duidelijkheid zouden brengen, maar niets was minder waar. Zowat elk artikel kwam met een andere oorzaak aanzetten: het gebruiken van sojaproducten, het nemen van antidepressiva, een zwangerschap, enzovoort. Omdat ik al snel merkte dat er nog niet veel duidelijkheid bestond over deze aandoening en haar oorzaken, besloot ik hiervan mijn missie te maken. Het hoofdopzet van mijn onderzoek bestond er uit om alle oorzaken van PGAD te verzamelen die tot hiertoe zijn verschenen binnen de literatuur.

Alvorens me specifiek te focussen op de oorzaken, moest ik me eerst nog wat verder verdiepen in de vrouwelijke anatomie en fysiologie. Want zelfs al studeer je seksuologie, over de vulva en de clitoris valt altijd nog wel iets bij te leren. Wist je trouwens dat een clitoris er zo uitziet ? 

Clitoris

Nadien ging ik me specifiek focussen op de aandoening: hoe vaak komt het voor? Wat zijn de symptomen? En komt het ook bij mannen voor? Volgens de literatuur treft PGAD ongeveer 1% van de vrouwelijke bevolking. Ook bij mannen zijn er patiënten bekend, maar in mindere mate. Wat de symptomen betreft zijn deze niet bij elke vrouw hetzelfde. Bij sommigen blijft de opwinding uren of zelfs dagen duren zonder dat deze verdwijnt bij het krijgen van een orgasme. Het gaat meestal om clitorale tintelingen, irritatie, een kloppende pijn, een drukkend gevoel en af en toe komen er ook spontane orgasmes bij kijken. 

De meest voorkomende locaties van de symptomen zijn de clitoris, de vagina, de schaamlippen of een combinatie van de drie. 

Maar hoe komen deze symptomen dan toch tot stand? Deze vraag probeer ik binnen mijn masterproef zo duidelijk mogelijk te beantwoorden aan de hand van een schema. Dit schema geeft een overzichtelijke samenvatting van alle oorzaken binnen de literatuur waarover consensus bestaat.

Voor iemand die niet helemaal bekend is met biologie en de werking van de hersenen ziet dit schema er misschien uit zoals een Spaans boek voor iemand die nog nooit eerder Spaans sprak. Geen zorgen, ik probeer het even in mensentaal uit te leggen. 

Schermafbeelding 2019-10-16 om 21.32.36 0Bij PGAD wordt de hypothalamus (een onderdeel van de hersenen) te sterk gestimuleerd door een teveel aan dopamine (het beloningshormoon) en dit kan verschillende oorzaken hebben. 

Eerst en vooral door zaken die rechtstreeks inwerken op de hypothalamus, zoals bijvoorbeeld het nemen van antidepressiva. Daarnaast kan het ook zijn dat de schaamzenuw (een zenuw in het bekken) verkeerde signalen uitstuurt naar de hypothalamus, door bijvoorbeeld de aanwezigheid van een cyste of de aanwezigheid van spataders in het bekken. Ten slotte kan er ook meer bloedstroom naar de geslachtsdelen optreden door een bloedvatmisvorming, de remslaap, chronisch vermoeidheidssyndroom of door het stoppen van antidepressiva.

De afbeelding hieronder geeft het proces op een vereenvoudigde manier weer. Bij PGAD wordt in de hypothalamus te veel dopamine vrijgezet. Hierdoor ontstaat genitale opwinding.

Maar hoe kan dit probleem nu verholpen worden? Er zijn enkele medicijnen op de markt die de dopaminesecretie verlagen waardoor ook de opwinding en andere symptomen vanPGAD zullen verdwijnen. 

Daarnaast is soms ook een oorzakelijke behandeling mogelijk. Zo is een chirurgische behandeling bij cysten effectief om de symptomen van PGAD te verlichten. Wat antidepressiva betreft kunnen de symptomen zelfs helemaal verdwijnen door het stopzetten van de medicatie.

Ook psychotherapie wordt aanbevolen om de impact van PGAD op het psychologische en seksuele welzijn te behandelen. Vrouwen met PGAD ondervinden namelijk vaak moeilijkheden met hun geestelijke gezondheid, hun dagelijkse activiteiten en met hun seksuele functioneren. PGAD wordt geassocieerd met hoge percentages van negatieve emoties, waaronder zorgen, stress en depressie. 

PGAD is met andere woorden een complexe aandoening met meerdere oorzaken waarbij het gaat om een samenspel van biopsychosociale factoren. Doordat de literatuur vooral wordt gekenmerkt door individuele casussen staat het onderzoek nog in zijn kinderschoenen. Er zijn nog veel vragen over de oorzaken, behandeling en de impact van PGAD die onbeantwoord blijven. 

Mijn model kan een mooie aanzet zijn voor verder onderzoek en kan seksuologen en artsen helpen om PGAD beter te begrijpen. Want wat mannen ook mogen beweren, een vrouw (en haar lichaam) begrijpen is zeker niet onmogelijk.
