Diep in onze longen bevindt zich een beschermlaag die ons elke dag verdedigt tegen stofdeeltjes, bacteriën en virussen: mucus. Wat voor gezonde mensen een onmisbare eerste verdedigingslinie is, vormt voor mensen met mucoviscidose een bijna ondoordringbare barrière. De taaie en kleverige mucus verhindert niet alleen de afvoer van ziekteverwekkers, maar houdt ook geneesmiddelen tegen nog voor ze hun doel bereiken. Net daar begint de uitdaging van dit onderzoek.
Heel wat veelbelovende geneesmiddelen zijn slecht oplosbaar in water. Daardoor lossen ze moeilijk op in het vocht dat de luchtwegen bekleedt en bereiken ze onvoldoende de plaats waar ze werkzaam moeten zijn. Apremilast is zo'n geneesmiddel. Hoewel het therapeutisch potentieel heeft voor de behandeling van luchtwegontstekingen bij mucoviscidose, vormt zowel de beperkte oplosbaarheid als de slijmbarrière een groot obstakel voor een doeltreffende luchtwegtoediening. De centrale vraag van dit onderzoek luidde daarom: kunnen apremilast‑nanokristallen diffunderen door gezonde en mucoviscidose‑mucusmodellen, en welke formulatie‑ en mucusparameters bepalen die diffusie?

Om die vraag te beantwoorden werd eerst een oplossing gezocht voor de slechte oplosbaarheid van apremilast. Het geneesmiddel werd verwerkt tot nanokristallen, ultrakleine deeltjes met een grootte van minder dan 250 nanometer. Door hun kleine afmetingen beschikken deze deeltjes over een veel groter oppervlak, waardoor ze sneller kunnen oplossen. De nanokristallen werden geproduceerd via een geminiaturiseerd wet bead milling-proces en gestabiliseerd met verschillende hulpstoffen: Lutrol F127, natriumcholaat of een combinatie van natriumcholaat en chitosan. Vervolgens werden hun grootte, homogeniteit, oppervlaktelading en structurele integriteit nauwkeurig onderzocht.

Een geneesmiddel ontwikkelen is echter slechts een deel van het verhaal. Even belangrijk is de omgeving waarin het geneesmiddel moet functioneren. Daarom werden in dit onderzoek ook synthetische mucusmodellen ontwikkeld die de eigenschappen van gezond en mucoviscidoseachtig luchtwegmucus zo nauwkeurig mogelijk nabootsen. De hydrogels werden uitgebreid gekarakteriseerd en vertoonden dezelfde typische kenmerken als echt mucus: gezonde modellen waren minder viskeus en elastisch, terwijl de mucoviscidosemodellen een veel dichter en sterker verknoopt netwerk vormden. Daarmee ontstond een reproduceerbaar modelsysteem waarin de verplaatsing van nanokristallen gecontroleerd kon worden bestudeerd.
De resultaten tonen aan dat niet de dikte van het slijm, maar vooral de interactie tussen de nanokristallen en de mucuscomponent mucine bepaalt hoe goed een geneesmiddel zich kan verplaatsen. De eerste zes uur na toediening bleken daarbij cruciaal. In die periode werd beslist hoe diep de nanokristallen uiteindelijk door de mucus konden dringen.
De verschillen tussen de formuleringen waren opvallend. Nanokristallen gestabiliseerd met Lutrol F127 bereikten consequent de grootste diffusie, zowel in gezonde als in mucoviscidoseachtige mucusmodellen. Hun gehydrateerde, muco-inerte oppervlak beperkte de interactie met mucine, waardoor ze zich relatief vrij door de mucus konden bewegen. Nanokristallen met natriumcholaat ondervonden meer weerstand door hun negatieve oppervlaktelading, terwijl de combinatie met chitosan leidde tot sterke elektrostatische bindingen met mucine. Die interacties hielden de nanokristallen als het ware gevangen in de mucus, waardoor hun transport sterk werd beperkt.
Daarmee toont dit onderzoek aan dat succesvolle mucuspenetratie niet uitsluitend afhangt van de eigenschappen van de mucus zelf, maar vooral van een doordacht ontwerp van het oppervlak van de nanokristallen. De keuze van de stabilisator bepaalt hoe het geneesmiddel zich gedraagt in een complexe biologische omgeving en kan het verschil maken tussen een formulering die haar doel bereikt en een formulering die onderweg vastloopt.
Naast de ontwikkeling van de nanokristallen leverde dit onderzoek ook een belangrijke bijdrage aan het onderzoeksveld van pulmonale geneesmiddeltoediening en mucus-gebaseerd geneesmiddelenonderzoek. De ontwikkelde synthetische mucusmodellen vormen een robuust, reproduceerbaar en afstembaar platform om nieuwe formuleringen in een vroeg stadium te evalueren. Daardoor kunnen onderzoekers verschillende strategieën systematisch vergelijken zonder voortdurend afhankelijk te zijn van patiëntmateriaal. Dat versnelt de ontwikkeling van nieuwe geneesmiddelen en maakt het mogelijk om formuleringen efficiënter te optimaliseren.
De belangrijkste conclusie is duidelijk. Apremilast-nanokristallen blijken niet alleen succesvol te produceren en te stabiliseren, maar zijn ook in staat om relevante slijmbarrières te doordringen. Vooral de formulering gestabiliseerd met Lutrol F127 komt naar voren als de meest veelbelovende kandidaat voor toekomstige pulmonale toediening. Tegelijk bewijzen de ontwikkelde mucusmodellen hun waarde als voorspellend systeem voor het gedrag van nanokristallen in zowel gezonde als mucoviscidoseachtige luchtwegen.
Dit onderzoek laat zien dat de oplossing voor een hardnekkig medisch probleem niet noodzakelijk ligt in een nieuw geneesmiddel, maar in een slimmere manier om dat geneesmiddel op zijn bestemming te krijgen. Door de onzichtbare slijmbarrière beter te begrijpen én te leren overwinnen, wordt de weg vrijgemaakt voor efficiëntere luchtwegtherapieën. Wat vandaag begint met nanokristallen van apremilast, kan morgen bijdragen aan een nieuwe generatie geneesmiddelen die eindelijk voorbij de grootste hindernis in de longen geraakt.
